数字减影血管造影术引导下神经介入治疗对老年缺血性脑卒中患者神经功能的影响

摘　　要：目的:分析数字减影血管造影术(DSA)引导下神经介入治疗对老年缺血性脑卒中患者神经功能的影响。方法:选择拟行手术治疗的108例老年缺血性脑卒中患者为研究对象,按照治疗方式不同将患者分为常规组和DSA组,每组各54例。常规组接受静脉溶栓基础治疗;DSA组在常规治疗基础上接受DSA引导下神经介入治疗。对比两组患者治疗后的血管再通率以及治疗前后的神经功能、血管内皮功能和日常活动能力等。结果:DSA组的血管再通率为79.63%,高于常规组的55.56%(P<0.05);治疗前,两组患者神经功能和日常活动能力评分及血管内皮功能指标比较,差异无统计学意义(P>0.05);治疗后,两组患者的神经功能、日常活动能力评分和血管内皮功能指标均优于治疗前,且DSA组优于常规组(P<0.05)。结论:DSA引导下神经介入治疗老年缺血性脑卒中的疗效确切,可有效提高患者的血管再通率,改善患者的神经功能和血管内皮功能,提高患者的日常活动能力。Objective:To analyze the effect of digital subtraction angiography(DSA)-guided Neurointerventional therapy on neurological function in elderly patients with ischemic stroke.Methods:108 elderly patients with ischemic stroke were divided into the conventional group and the DSA group according to different treatment methods,54 cases in each group.The conventional group received intravenous thrombolytic therapy,while the DSA group received DSA guided nerve intervention therapy on the basis of conventional therapy.The recanalization rate of blood vessels,nerve function,vascular endothelial function and daily activity ability before and after treatment were compared between the two groups.Results:The recanalization rate of DSA group was 79.63%,which was higher than 55.56%of the conventional group(P<0.05).There was no significant difference in the neurological function,daily activity ability score and vascular endothelial function index between the two groups before treatment(P>0.05).After treatment,the neurological function,daily activity ability score and vascular endothelial function index of the two groups were better than before treatment,and the DSA group was better than the conventional group(P<0.05).Conclusion:DSA guided nerve intervention in the treatment of elderly patients with ischemic stroke is effective,which can effectively improve the vascular recanalization rate,improve the neurological function and vascular endothelial function,and improve the daily activity ability of patients.
