摘 要:牙周炎是口腔常见的慢性感染性疾病,可造成牙周支持组织的破坏并最终导致牙齿丧失。牙周组织健康与否在影响全身多系统健康的同时,也受到多种系统性疾病的影响。胆汁酸是由肝脏合成并以胆汁形式排泄到小肠中的一类重要化合物,不仅参与脂质和脂溶性维生素的吸收过程,还能够激活多种核受体和膜受体介导的信号通路,参与糖脂代谢的精细调节过程;此外,胆汁酸还能影响肠道菌群的构成以及激素的合成与分泌。胆汁酸代谢失衡及信号传导异常已被证实与多种系统性疾病密切相关,但目前,对胆汁酸影响牙周组织健康的研究还较为粗浅。本综述首先总结了胆汁酸对多种影响牙周健康的系统性疾病的影响,进一步深入探究了胆汁酸对骨代谢、免疫细胞与口腔菌群等直接影响牙周健康的因素的影响,并探寻胆汁酸对牙周组织健康影响的可能机制与干预靶点,旨在为临床牙周炎疾病的诊断、治疗与管理提供有益的理论依据。Periodontitis is a common chronic infectious disease in the oral cavity,which can cause significant destruction of periodontal supporting tissues and tooth loss.The health of periodontal tissues not only affects the health of multiple systems throughout the body,but is also regulated by various systemic diseases.Bile acids are an important class of compounds synthesized by the liver and excreted into the small intestine in the form of bile.They not only participate in the absorption of lipids and fat-soluble vitamins,but also activate multiple nuclear receptor and membrane receptor-mediated signaling pathways to participate in the fine regulation of glucose and lipid metabolism.Additionally,can affect the composition of the intestinal microbiota and the synthesis and secretion of certain hormones.Imbalances in metabolism and abnormal signal transmission have been shown to be closely related to various systemic diseases.However,current research on the impact of bile acids on periodontal tissue health is still relatively superficial.This review first summarizes the types and basic characteristics of bile acids and further analyzes their impact on various systemic diseases closely related to periodontitis.Simultaneously,it delves into the direct impact of bile acids on factors that affect periodontal health,such as bone metabolism,immune cells,and oral microbiota,to explore potential mechanisms and intervention targets for the effect of bile acids on periodontal tissue health.This review aims to provide a useful theoretical basis and solid foundation for the diagnosis,treatment,and management of clinical periodontitis diseases.
