粪便隐血联合肿瘤标志物CEA、TPA、CYFRA211、CA125、CA199诊断结直肠癌的临床价值

摘　　要：目的:探讨粪便隐血联合肿瘤标志物癌胚抗原(CEA)、组织多肽抗原(TPA)、细胞角蛋白19的可溶性片段(CYFRA211)、糖类抗原125(CA125)、糖类抗原199(CA199)诊断结直肠癌的临床价值。方法:选取2021年1月至2023年12月我院收治的87例结直肠癌患者作为研究组,选取同期于我院体检的87名健康志愿者作为对照组。比较2组受试者粪便隐血检查结果及血清CEA、TPA、CYFRA211、CA125、CA199水平;分析各指标单项与联合检测对结直肠癌的诊断效果。结果:研究组粪便隐血阳性率(74.71%)显著高于对照组(5.75%),P<0.05。研究组血清CEA、TPA、CYFRA211、CA125、CA199水平亦显著高于对照组,P<0.05。ROC曲线分析得出,粪便隐血、CEA、TPA、CYFRA211、CA125、CA199、联合诊断结直肠癌的AUC分别为0.845、0.745、0.755、0.876、0.779、0.758、0.972,敏感度分别为74.7%、50.6%、50.6%、77.0%、56.3%、100%、95.4%,特异度分别为94.3%、93.1%、100%、100%、100%、50.6%、96.6%。结论:结直肠癌患者粪便隐血阳性率高,且血清CEA、TPA、CYFRA211、CA125、CA199呈高表达,各指标联合检测可提高对结直肠癌的诊断效果。Objective To investigate the clinical value of fecal occult blood(OB)combined with tumor markers carcinoembryonic antigen(CEA),tissue polypeptide antigen(TPA),soluble fragment of cytokeratin 19(CYFRA211),carbohydrate antigen 125(CA125)and carbohydrate antigen 199(CA199)in the diagnosis of colorectal cancer.Methods Total 87 patients with colorectal cancer admitted to our hospital from January 2021 to December 2023 were selected as study group,and 87 healthy volunteers who underwent physical examination in our hospital during the same period were selected as control group.The results of fecal OB test and serum levels of CEA,TPA,CYFRA211,CA125,CA199 were compared between the two groups to analyze the diagnostic effect of single and combined indexes on colorectal cancer.Results The positive rate of fecal OB in the study group(74.71%)was significantly higher than that in the control group(5.75%)(P<0.05).Serum levels of CEA,TPA,CYFRA211,CA125 and CA199 in the study group were significantly higher than those in the control group(P<0.05).ROC curve analysis showed that the AUC of fecal OB,CEA,TPA,CYFRA211,CA125,CA199 and combined diagnosis of colorectal cancer were 0.845,0.745,0.755,0.876,0.779,0.758 and 0.972,respectively.Sensitivity was 74.7%,50.6%,50.6%,77.0%,56.3%,100%,95.4%,and specificity was 94.3%,93.1%,100%,100%,100%,50.6%,96.6%,respectively.Conclusion The positive rate of fecal OB in patients with colorectal cancer is high,and the serum CEA,TPA,CYFRA211,CA125,CA199 are highly expressed.The combined detection of all indicators can improve the diagnostic effect of colorectal cancer.
