摘　　要：目的探讨D型人格与前循环急性缺血性脑卒中患者预后的相关性。方法回顾性分析2022年9月至2023年7月在中山大学附属第五医院接受治疗的前循环急性缺血性脑卒中患者资料。采用D型人格量表(D-type personality scale,DS-14)评估参与者的消极情感(negative affectivity,NA)和社会抑制(social inhibition,SI)维度,使用研究样本中每个维度的中位数(NA≥11且SI≥9)来评估中风患者是否为D型人格。90 d改良Rankin量表(modified Rankin scale,mRS)评分3~6分定义为不良功能预后。采用单因素和多因素Logistic回归分析明确D型人格与90 d不良预后的相关性。结果共计纳入了36例患者,其中16例患者是D型人格。D型人格与非D型人格患者在90 d不良预后上,差异有统计学意义(P=0.019)。多因素分析显示,D型人格(OR=21.837,95%CI:1.287~370.557;P=0.033)、NA(OR=1.208,95%CI:1.006~1.451;P=0.043)、SI(OR=1.187,95%CI:1.012~1.392;P=0.035)是前循环急性缺血性脑卒中患者90 d不良功能预后的独立危险因素。结论D型人格可能与前循环急性缺血性脑卒中患者90 d不良预后独立相关。Objective To explore the association between D-type personality and the 90-day prognosis of acute ischemic stroke patients with anterior circulation.Methods A retrospective analysis was conducted on the data of patients with anterior circulation acute ischemic stroke who received treatment at the Fifth Affiliated Hospital of Sun Yat sen University from September 2022 to July 2023.The D-type personality scale(DS-14)was used to evaluate the negative affectivity(NA)and social inhibition(SI)dimensions of participants,and the median of each dimension in the study sample(NA≥11 and SI≥9)was used to assess whether stroke patients have a D-type personality.A score of 3-6 on the 90 day modified Rankin scale(mRS)is defined as poor functional prognosis.Using univariate and multivariate logistic regression analysis to clarify the correlation between D-type personality and poor prognosis at 90 days.Results: A total of 36 patients were enrolled in this study,of whom 16(44%)patients had a D-type personality tendency.There is a statistically significant difference(P=0.019)in poor prognosis between patients with type D personality and those without type D personality at 90 days.Multivariate analysis showed that D-type personality(OR=21.837,95%CI:1.287~370.557;P=0.033),NA(OR=1.208,95%CI:1.006~1.451;P=0.043),SI(OR=1.187,95%CI:1.012~1.392;P=0.035)is an independent risk factor for 90 day poor functional prognosis in patients with anterior circulation acute ischemic stroke.Conclusion D-type personality is independently associated with poor 90-day prognosis in patients with acute ischemic stroke in the anterior circulation.
