尼莫地平片联合天麻素对脑动脉粥样硬化的应用效果及相关代谢产物水平影响

摘　　要：目的探讨尼莫地平片联合天麻素对脑动脉粥样硬化(ACI)患者的临床疗效,并分析患者治疗前后相关代谢产物的水平变化.方法前瞻性选取天津港口医院2022年1月-2024年1月收治的80例ACI患者展开研究,应用抽签法分为观察组与对照组,每组各40例.对照组采取常规西医治疗,观察组增加尼莫地平片联合天麻素治疗.对比两组患者临床疗效,治疗前后脑组织相关代谢产物,血管内皮功能相关指标,血流动力学指标变化,并对比其不良反应发生率.结果观察组总有效率高于对照组(P<0.05);治疗后两组患者乳酸(Lac)含量降低,观察组低于对照组,肌酸(Cr)、胆碱(Chol)及N-乙酰天门冬氨酸(NAA)含量升高,观察组高于对照组(P<0.05);治疗后两组内皮素-1(ET-1)水平均降低,观察组低于对照组,一氧化氮(NO)水平升高,观察组高于对照组(P<0.05);治疗后两组红细胞比容、血浆黏度、全血黏度均降低,且观察组低于对照组(P<0.05);两组不良反应发生率对比无明显差异(P>0.05).结论尼莫地平片联合天麻素对ACI治疗效果显著,可改善患者脑组织相关代谢物水平,辅助改善血管内皮功能,降低血液黏度,且安全性较高.Objective To explore the clinical effect of nimodipine tablets combined with gastrodin on patients with cerebral atherosclerosis(ACI),and analyze the changes of related metabolites before and after the data.Methods A prospective study was conducted on 80 ACI patients admitted to Tianjin Port Hospital from January 2022 to January 2024.They were divided into an observation group and a control group using a lottery method,with 40 patients in each group.The control group received routine Western medicine treatment,while the observation group received additional treatment with nimodipine tablets combined with gastrodin.Compare the clinical efficacy,changes in brain tissue related metabolites,vascular endothelial function related indicators,and hemodynamic indicators between two groups of patients before and after treatment,and compare the incidence of adverse reactions.Results The effective overall rate of the observation group was higher than that of the control group(P<0.05);Lactate(Lac)levels decreased in both patient groups after treatment,while the observation group was lower than the control group.Creatine(Cr),choline(chol)and N-acetylamartinic acid(NAA)concentrations increased while the observation group was higher than the control group(P<0.05);after treatment,endothelin-1(ET-1)decreased in both patient groups and the observation group was lower than in the control group.Nitrogen oxide(NO)levels increased and the observation group was higher than the control group(P<0.05);after treatment,haematocrit,plasma and whole blood viscosity decreased in both patient groups and the observation group was lower than the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Nimodipine tablet combined with gastrodin has significant therapeutic effect on cerebral atherosclerosis,which can improve the level of related metabolites in patients'brain tissue,assist in improving vascular endothelial function,reduce blood viscosity,and has high safe
