胃癌患者血清中高尔基体蛋白73、claudin-1的表达水平及其临床意义

摘　　要：目的探讨胃癌患者血清中高尔基体蛋白73、claudin-1表达水平及其临床意义。方法采用酶联免疫吸附法(ELISA)检测76例胃癌患者(胃癌组)和30例健康体检者(对照组)血清中的GP73、claudin-1水平,比较2组的血清GP73、claudin-1水平差异并经受试者工作特征(ROC)曲线评估血清GP73、claudin-1水平鉴别诊断胃癌的临床效能,分析血清GP73、claudin-1水平与胃癌临床病理特征和总生存期(overall survival,OS)、无进展生存期(progression-free survival,PFS)的关系。结果胃癌组血清GP73、claudin-1水平低于对照组,差异均具有统计学意义(P<0.05)。ROC曲线分析显示,对胃癌的诊断,GP73最佳截断值21.18μg/L,敏感度83.20%、特异度85.73%;claudin-1最佳截断值0.72 ng/ml,敏感度82.16%、特异度84.44%。依据血清GP73、claudin-1水平截断值分别分成高、低表达组,GP73高表达组、claudin-1高表达组与GP73低表达组、claudin-1低表达组在浸润深度、分化程度、肿瘤大小、组织学类型、TNM分期方面比较,差异均具有统计学意义(P<0.05)。生存分析显示,GP73低表达组总生存率(28.21%)低于GP73高表达组总生存率(59.46%)(P<0.05),GP73低表达组无进展生存率(20.51%)低于GP73高表达组无进展生存率(40.54%)(P<0.05);claudin-1低表达组总生存率(31.58%)低于claudin-1高表达组总生存率(57.89%)(P<0.05),claudin-1低表达组无进展生存率(21.05%)低于claudin-1高表达组无进展生存率(36.84%)(P<0.05)。结论胃癌患者血清GP73水平、claudin-1水平均明显下降,且均与恶性病理特征和不良预后有关,可作为潜在的鉴别诊断胃癌的血清肿瘤学指标。Objective To investigate the expression levels of serum Golgi protein 73 and claudin-1 in patients with gastric cancer and their clinical significance.Methods Serum levels of GP73 and claudin-1 in 76 patients with gastric cancer(gastric cancer group)and 30 healthy subjects(control group)were detected by enzyme-linked immunosorbent assay(ELISA).The difference of serum GP73 and claudin-1 levels between the two groups was compared,and the clinical efficacy of serum GP73 and claudin-1 levels in differential diagnosis of gastric cancer was evaluated by receiver operating characteristic(ROC)curve.The relationship between serum levels of GP73 and claudin-1 and clinicopathological features,Overall Survival(OS)and Progression-Free Survival(PFS)of gastric cancer was analyzed.Results The levels of serum GP73 and claudin-1 in gastric cancer group were lower than those in control group,and the differences were statistically significant(P<0.05).ROC curve analysis showed that the optimal cut-off value of GP73,sensitivity 83.20%and specificity 85.73%were 21.18μg/L for the diagnosis of gastric cancer.The best cut-off value of claudin-1 was 0.72 ng/ml,the sensitivity was 82.16%,and the specificity was 84.44%.According to the cutoff values of serum GP73 and claudin-1 levels,they were divided into high and low expression groups.GP73 high expression group,claudin-1 high expression group and GP73 low expression group,claudin-1 low expression group had statistically significant differences in invasion depth,differentiation degree,tumor size,histological type and TNM stage(P<0.05).Survival a-nalysis showed that the overall survival rate(28.21%)in the low GP73 expression group was lower than that in the high GP73 expression group(59.46%)(P<0.05),and the progression-free survival rate(20.51%)in the low GP73 expression group was lower than that in the high GP73 expression group(40.54%)(P<0.05).The overall survival rate of low claudin-1 expression group(31.58%)was lower than that of high claudin-1 expression group(57.89%)(P<0.05).The progr
