不同分级异位萌出第一恒磨牙治疗后的疗效评估及相关影响因素分析

摘　　要：目的:探讨不同分级异位萌出第一恒磨牙分别使用不同的矫治方法治疗的效果。方法:选择河南省儿童医院口腔科矫治第一恒磨牙异位萌出的患者20名,10例患者第一恒磨牙异位萌出分级程度为Ⅰ级或Ⅱ级,采用分牙皮圈或分牙簧进行矫治;10例患者第一恒磨牙分级程度为Ⅲ级或Ⅳ级,应用改良Halterman矫治器技术。分析两组年龄、性别及治疗时间的差异,测量不同分级异位萌出的第一恒磨牙矫治前后磨牙近中倾斜角度的变化,并与相关影响因素进行分析。结果:分牙组平均矫治年龄为7.3岁,矫治器组平均年龄为8.2岁,两组治疗时间最长为150 d,最短60 d,分牙组患者平均矫治时间为87 d,矫治器组平均矫治时间为125 d,两组年龄及治疗时间差异均有统计学意义。两组中男性患儿占比均高于女性患儿,两组治疗前后第一恒磨牙平均近中倾斜角度差值均有统计学意义。结论:使用分牙法和改良Halterman矫治器治疗异位萌出第一恒磨牙均能获得良好的治疗效果,第一恒磨牙异位萌出不同分级程度可能与年龄、治疗时间差异及治疗前后第一恒磨牙近中倾斜角度差值有关。AIM:To investigate the effects of different grades of ectopic eruption of first permanent molarstreated with different orthodontic methods.METHODS:A total of 20 patients with ectopic eruption of the first permanent molars were selected from the Department of Stomatology of Henan Children’s Hospital,and the degree of ectopic eruption of the first permanent molars in 10 patients was graded as grade I or II,and the orthodontic rings or springs were used for orthodontic treatment.In 10 patients,the first permanent molars were graded as grade III or IV,and modified Halterman appliance was applied.The differences in age,gender,and treatment time between the two groups were analyzed.The changes in the mesial inclination angle of the molars before and after orthodontic treatment of the first permanent molars erupted by different grades of ectopic eruption were measured.The correlation analysis was carried out with the variables.RESULTS:The average age of orthodontic treatment in the dental group was 7.3 years,the average age of the orthodontic group was 8.2 years,the longest treatment time was 150 days and the shortest was 60 days,the average orthodontic time of the patients in the dental group was 87 days,and the average orthodontic time of the orthodontic group was 125 days.The proportion of male children in both groups was higher than that of female children.There was a statistically significant difference in the average mesial inclination angle of the first permanent molars before and after treatment in the two groups.CONCLUSION:The treatment of ectopic erupted first permanent molars with tooth segmentation and removable appliances can achieve good results,and the different grading degrees may be related to the difference in age,treatment time,and the angle of the first permanent molars before and after treatment.
