低能量激光辅助盐酸米诺环素治疗种植体周围黏膜炎疗效观察

摘　　要：目的评价低能量激光辅助盐酸米诺环素治疗对种植体周围黏膜炎的临床疗效。方法选取康复大学青岛医院(青岛市市立医院)2021年1月至2023年2月期间收治的种植体周围黏膜炎患者52例作为研究对象,其中男29例,女23例,平均年龄(51.35±11.82)岁。将患者随机分为对照组及试验组,每组各26例。对照组采用机械治疗+盐酸米诺环素治疗;试验组采用机械治疗+盐酸米诺环素治疗+低能量激光照射。在治疗前及治疗后第1、3、6个月时对各组患者的改良菌斑指数(mPLI)、牙龈指数(GI)、改良龈沟出血指数(mSBI)、种植体周围袋探诊深度(PPD)进行测定并比较,在治疗前及治疗后第6个月时对各组患者龈沟液中白细胞介素(IL)-6、IL-1β、肿瘤坏死因子-α(TNF-α)进行测定并比较。结果治疗后第1、3个月,试验组mPLI、GI、mSBI、PPD均低于对照组,差异有统计学意义(P<0.05)。治疗后第6个月,试验组mPLI及mSBI低于对照组,差异有统计学意义(P<0.05),试验组与对照组GI及PPD比较,差异无统计学意义(P>0.05)。治疗后第6个月试验组IL-6、IL-1β、TNF-α水平均低于对照组,差异有统计学意义(P<0.05)。结论低能量激光辅助盐酸米诺环素在短期内对种植体周围黏膜炎治疗效果良好,可降低炎性细胞因子水平,改善牙周状况,值得临床推广。Objective To evaluate the clinical effect of low-energy laser-assisted minocycline hydrochloride treatment on periimplant mucositis.Methods A total of 52 patients with peri-implant mucositis who were admitted to Qingdao Hospital of Rehabilitation University(Qingdao Municipal Hospital)from January 2021 to February 2023 were selected as the research objects,including 29 males and 23 females with an average age of(51.35±11.82)years.Patients were randomly divided into control group and experimental group with 26 patients in each group.Control group was treated with mechanical therapy and minocycline hydrochloride;experimental group was treated with mechanical therapy+minocycline hydrochloride therapy+low-energy laser irradiation.The modified plaque index(mPLI),gingival index(GI),modified gingival sulcus bleeding index(mSBI)and probing depth around implants(PPD)were measured and compared at baseline and at 1,3 and 6 months after treatment.IL-6,IL-1βand TNF-αin gingival crevicular fluid of patients in each group were measured at baseline and at 6months after treatment.Results The mPLI,GI,mSBI and PPD in the experimental group were lower than those in the control group at 1 and 3 months after treatment,and the differences were statistically significant(P<0.05).The mPLI and mSBI in the experimental group were lower than those in the control group at the 6 months after treatment,and the differences were statistically significant(P<0.05);while GI and PPD were lower but did not show statistically significant differences(P>0.05).The levels of IL-6,IL-1βand TNF-αin the experimental group were lower than those in the control group at the 6 months after treatment,and the differences were statistically significant(P<0.05).Conclusions Low-energy laser-assisted minocycline hydrochloride has a good therapeutic effect on peri-implant mucositis in the short term,reducing inflammatory cytokines expression and improving periodontal conditions.The approach is worth clinical promotion.
