摘 要:目的:探究精神分裂症(schizophrenia,ES)患者血清成纤维细胞生长因子22(FGF22)、胃促生长素(ghrelin)水平与认知功能关系以及诊断价值。方法:选取2021年8月-2023年8月在某院治疗的ES患者128例作为研究组,另选择同期在同院体检健康者128例作为对照组。并且根据有无认知功能损害分为障碍组(n=50)和正常组(n=78)。采用酶联免疫吸附(ELISA)法检测受试者血清FGF22、ghrelin水平;Spearman法分析ES患者血清FGF22、ghrelin与PANSS评分、MCCB 10项测试评分、Stroop色词3项测试评分相关性;多元Logistic回归分析ES发生的影响因素;受试者工作特征(ROC)曲线分析血清FGF22、ghrelin对ES的诊断价值。结果:血清FGF22、ghrelin在研究组中的表达水平均低于对照组(t=9.837,11.166;P<0.05);血清FGF22、ghrelin在障碍组中的表达水平均低于正常组(t=5.967,10.254;P<0.05);研究组PANSS总分为84.17±8.32分。研究组MCCB 10项测试评分、Stroop色词3项检测评分均小于对照组(P<0.05);Spearman法分析得出,血清FGF22、ghrelin均与PANSS评分呈负相关(P<0.05),与MCCB 10项测试评分、Stroop色词3项检测评分呈正相关(P<0.05);多元Logistic回归分析得知血清FGF22、ghrelin均是影响ES发生的保护因素(P<0.05);ROC曲线得出,血清FGF22诊断ES的AUC为0.815,血清ghrelin诊断ES的AUC为0.835,二者联合诊断ES的AUC为0.911,二者联合优于各自单独诊断(Z联合vs FGF22=3.009,Z联合vs ghrelin=2.514;P<0.05)。结论:ES患者血清FGF22、ghrelin水平均表达下调,与认知功能密切相关,且两者联合具有较高的ES诊断价值。Objective:To investigate the relationship between serum fibroblast growth factor 22(FGF22)and ghrelin levels and cognitive function in patients with schizophrenia(ES)and their diagnostic value.Methods:A total of 128 ES patients treated in our hospital from August 2021 to August 2023 were selected as the study group,and 128 healthy patients were selected as the control group during the same period.They were grouped into impairment group(n=50)and normal group(n=78)according to the presence or absence of cognitive impairment.Enzyme-linked immunosorbent assay(ELISA)was used to measure serum FGF22 and ghrelin levels.Spearman method was used to analyze the correlation between serum FGF22 and ghrelin and PANSS score,scores of the 10-item MCCB test and the scores of 3-item Stroop color word test in ES patients.Multivariate logistic regression was used to analyze the influencing factors of ES occurrence.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of serum FGF22 and ghrelin for ES.Results:The expression levels of serum FGF22 and ghrelin in the study group were lower than those in the control group(t=9.837,11.166;P<0.05).The expression levels of serum FGF22 and ghrelin in the impairment group were lower than those in the normal group(t=5.967,10.254;P<0.05).The total score of PANSS in the study group was(84.17±8.32).The scores of the 10-item MCCB test and the scores of 3-item Stroop color word test in the study group were lower than those in the control group(P<0.05).Spearman analysis showed that serum FGF22 and ghrelin were negatively correlated with PANSS score(P<0.05),and positively correlated with scores of the 10-item MCCB test and the scores of 3-item Stroop color word test(P<0.05).Multivariate Logistic regression analysis showed that serum FGF22 and ghrelin were protective factors affecting the occurrence of ES(P<0.05).ROC curves showed that the AUC of serum FGF22 for the diagnosis of ES was 0.815,the AUC of serum ghrelin for the diagnosis of ES was 0.835,and the AUC of the tw
