摘　　要：目的 探讨应用局部滑行皮瓣修复拇指末节远端指腹小汗腺汗孔癌的临床效果。方法 患者经门诊取病理活检,确诊为左拇指小汗腺汗孔癌,择期行病灶扩大切除术,邻近皮瓣修复术。术中送即时冰冻切片病理,确认原发病灶切除彻底。设计左拇指指腹滑行皮瓣,修复缺损区创面。结果 术后伤口Ⅰ期愈合,皮瓣颜色、质地与周围皮肤基本一致。术后1个月首次复诊,切口瘢痕无明显增生,患指感觉及运动功能逐步恢复正常。结论 利用滑行皮瓣修复手指远端皮肤肿瘤术后缺损,就近取材,血运可靠,皮瓣易于成活,术后患指外观接近正常,无功能障碍,是临床上修复手指远端皮肤缺损的有效方法。Objective To evaluate the clinical efficacy of local sliding skin flap for the reconstruction of the distal pulp defect resulting from the excision of eccrine porocarcinoma in the terminal phalanx of the thumb.Methods The patient was confirmed the diagnosis of eccrine porocarcinoma in the left thumb by pathological.Subsequently,the patient was admitted for an expanded excision of the lesion,followed by reconstruction using an adjacent sliding skin flap.Intraoperatively,a frozen section analysis confirmed the complete removal of the primary lesion.A sliding skin flap was designed for the left thumb pulp to cover the defect.Results The postoperative wound healed primarily,and the color and texture of the flap closely matched the normal skin.At the first follow-up 1 month after surgery,the scar showed no significant hypertrophy,and the sensory and motor functions gradually returned to normal.Conclusion The utilization of sliding skin flaps for the reconstruction of distal finger skin defects after tumor excision is an effective technique.Its advantages include the proximity of the tissue source,reliable blood supply,and a high rate of flap survival.The postoperative appearance and functionality of the treated finger are near-normal,without functional impairment,so it is a valuable surgical option for wider clinical application worthy.
