摘 要:目的检测食管鳞癌患者放疗前、中、后口腔牙龈卟啉单胞菌(Porphyromonas gingivalis,Pg)的变化,探讨放射治疗对口腔Pg的清除作用。方法30例经病理证实的食管鳞癌患者,采用Elekta-precise计划系统制定适形调强放疗计划,处方剂量为50~50.4 Gy/25~28次,放疗前、中、后每周1次收集患者口腔拭子(每人6份,共180份),用qPCR法进行Pg定量检测。统计学数据采用秩和检验分析。结果30例食管鳞癌患者初始口腔Pg检测结果为:60%(18/30)为阴性,40%(12/30)为阳性,其中12例阳性患者在放疗结束后83.33%(10/12)转为阴性,16.67%(2/12)仍为阳性(2例未转阴患者口腔Pg量较放疗前明显减少(P<0.05)。结论食管鳞癌患者在接受食管肿瘤部位放射治疗过程中,口腔Pg可被同时清除或减少。Objective To detect the changes of oral Pg before,during and after radiotherapy in patients with esophageal cancer,and to explore the role of radiotherapy on oral Pg clearance.Methods Thirty patients with pathologically proven esophageal squamous cell carcinoma were selected for the formulation of a modulated intensity modulated radiotherapy plan using the Elekta-precise planning system.The prescribed dose was 50~50.4 Gy/25~28 times.Oral swabs(6 per person,180 in total)were collected once a week before,during and after radiotherapy.Quantitative detection of Pg was performed by qPCR.Statistical data were analyzed by rank sum test.Results The results of initial oral Pg detection in 30 patients with esophageal cancer were as follows:60%(18/30)were negative and 40%(12/30)were positive.Among the 12 positive patients,83.33%(10/12)were negative and 16.67%(2/12)were still positive after radiotherapy(for the two patients,the amount of Pg was significantly reduced compared with that before radiotherapy(P<0.05).Conclusion Patients with esophageal cancer,oral Pg can be simultaneously eliminated or reduced during radiotherapy on the cancer.
