摘　　要：目的探讨肺癌患者外周血循环肿瘤细胞(CTC)与肿瘤转移及新辅助化疗(NCT)疗效的关系。方法将106例肺癌患者按外周血CTC水平的不同分为阳性组(CTC≥5/7.5 ml)48例和阴性组(CTC﹤5/7.5 ml)58例。比较两组患者肿瘤转移情况、NCT疗效,采用Logistic回归模型分析肺癌患者外周血CTC水平的影响因素。结果阳性组患者淋巴结转移率、远处转移率均高于阴性组,疾病控制率低于阴性组,差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析结果显示,淋巴结转移情况、远处转移情况、疗效均是肺癌患者外周血CTC水平的独立影响因素(P﹤0.05)。结论肺癌患者外周血CTC水平与肿瘤转移情况和NCT疗效有关,对肺癌患者的预后预测有着一定的参考价值。Objective To investigate the relationship between peripheral blood circulating tumor cell(CTC)and tumor metastasis and the efficacy of neoadjuvant chemotherapy(NCT)in patients with lung cancer.Method A total of 106 patients with lung cancer were divided into positive group(48 cases)(CTC≥5/7.5 ml)and negative group(58 cases)(CTC<5/7.5 ml)according to different levels of peripheral blood CTC.The tumor metastasis and NCT efficacy of the two groups were compared,and the influencing factors of CTC level in peripheral blood of patients with lung cancer were analyzed by Logistic regression model.Result The lymph node metastasis rate,distant metastasis rate were higher and disease control rate was lower in positive group than those in negative group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that lymph node metastasis,distant metastasis and efficacy were independent influencing factors for CTC level in peripheral blood of lung cancer patients(P<0.05).Conclusion The level of CTC in peripheral blood of lung cancer patients is related to tumor metastasis and NCT efficacy,which has certain reference value in predicting the prognosis of lung cancer patients.
