数字化印模在后牙种植冠修复适合性的临床研究

摘  要:目的:采用硅橡胶复制法比较两种印模技术制作的后牙种植冠适合性。方法:选择2021年10月~2022年12月合肥市口腔医院收治的单颗后牙缺失患者102例,种植术后随机分为对照组和观察组,分别采用传统聚醚硅橡胶印模和数字化印模制作螺丝固位种植单冠。应用硅橡胶复制法比较两组牙冠的边缘适合性,并比较牙冠美国公共卫生署(USPHS)评分、取模及调改耗时、患者视觉模拟量表(VAS)评分以分析两种印模技术在种植单冠修复中的临床效果差异。结果:观察组种植单冠颊、舌侧边缘粘接层厚度小于对照组(P<0.05)。两组牙冠USPHS评分相近且均未出现C级评分,差异无统计学意义(P>0.05)。观察组取模及调改总耗时低于对照组(P<0.05),VAS评分高于观察组(P<0.05)。结论:两种印模技术制作的种植单冠适合性均达到临床标准。数字化印模耗时更短且患者舒适性好,可广泛应用于临床。Objective:To compare the fitness of posterior implant-supported crowns fabricated by two impression techniques using the silicone rubber replication method.Methods:One hundred and two patients with single posterior tooth loss treated at Hefei Stomatological Hospital were enrolled from October 2021 to December 2022,and randomly divided into control group and observational group after implant surgery.Traditional polyether silicone rubber impression and digital impression were used to fabricate screw-retained single implant crowns,respectively.The silicone rubber replication method was used to compare the marginal fit of the crowns in two groups.The clinical effectiveness of the two impression techniques in single implant crown restoration was analyzed by assessing the USPHS scores of the crowns,the duration of impression taking and adjustment,and patients′VAS scores.Results:The adhesive layer thickness at the buccal and lingual margins of the implant-supported single crowns in the observational group was significantly less than that in the control group(P<0.05).The USPHS scores for the crowns in both groups were similar,with no Grade C scores observed,and the difference was insignificant(P>0.05).The duration of impression taking and adjustment in the observational group was significantly lower than that in the control group(P<0.05).Patients′VAS scores for impression taking in the observational group were significantly higher than those in the control group(P<0.05).Conclusion:The fitness of implant-supported single crowns fabricated using both impression techniques meets clinical standards.Digital impression taking is worthy of wider clinical recommendation because it is faster and more comfortable for patients.
