甲下鲍恩病

摘　　要：报告1例甲下鲍恩病。患者男,28岁。1年前右手拇指甲远端出现纵向褐色条纹。皮肤科检查:右手拇指甲远端一纵向褐色条纹,约5 mm×1 mm,远端逐渐变细,至距甲游离缘约2 mm处消失;甲板表面光滑,无触压痛,甲周皮肤正常。皮肤镜检查:右手拇指甲甲板见纵向褐色条纹,远端甲下见黄白色角质性赘生物,局部甲床受压后呈凹陷状。皮损组织病理检查:表皮棘层肥厚,细胞全层排列紊乱,有异形,可见核分裂象和角化不良细胞;真皮浅层可见炎性细胞浸润。诊断:甲下鲍恩病。A case of subungual Bowen's disease is reported.A 28-year-old male presented with a longitudinal brown streak on the distal right thumb for one year.Dermatology examination showed a longitudinal brown streak at the distal end of the right thumbnail,sizing about 5 mmxl mm.The streak gradually tapered at the distal end and disappeared at about 2 mm from the free edge of the nail.The surface of nail plate was smooth,without tenderness and abnormal skin around the nail.Dermoscopy revealed a longitudinal brown streak on the right thumbnail plate,yellow white keratinous neoplasm growth under the distal part of the nail.Partial nail bed was depressed after compression.Histopathological examination showed hypertrophy of epidermal spinous layer,disordered arrangement of cells in the whole layer.There were cells with atypia,nuclear division and dyskerato-sis.Inflammatory cell infiltration in the superficial dermis was observed.A diagnosis of subungual Bowen's disease was made.
