摘　　要：报告1例恶性黑棘皮病。患者男,24岁。全身褶皱部位皮肤变黑、增厚2个月。皮肤科检查:全身褶皱部位皮肤色素加深、粗糙,呈天鹅绒样增厚,以颈部、腋下及会阴部为重;腋下散在乳头状赘生物。右侧锁骨上可触及一增大淋巴结。胃镜示胃窦、胃体、十二指肠球部壁黏膜粗糙、充血、水肿、糜烂,可见散在溃疡。胃部组织病理检查示低分化腺癌。胸部及腹部CT平扫+增强检查提示多发转移的可能性大。诊断:恶性黑棘皮病;胃低分化腺癌。A case of malignant acanthosis nigricans is reported.A 24-year-old male presented with blackened and thickened skin in the whole-body folds for two months.Physical examination showed the patient's skin in the folds of the whole body was deeply pigmented,rough,and velvet-like thickening,most obvious in the neck,armpit and perineum,and the underarm was scattered with papillary growths.An enlarged lymph node was touched on the right supraclavicular.Endoscopic examination re-vealed rough,hyperemic,edema and erosions in the wall of gastric antrum,gastric body and duodenum,with scattered ulcers,and histopathological examination indicated the presence of poorly differentiated adenocarcinoma.Chest and abdominal CT scan+contrast examination suggested the possibility of multiple metastases.The diagnosis of malignant acanthosis nigricans and gastric poorly differentiated adenocarcinoma were made.
