摘 要:目的:调查不孕症女性宫颈人乳头瘤病毒(human papillomavirus,HPV)的感染情况、亚型分布特点及与普通女性的差异,以期为制定特殊人群宫颈癌筛查及防治策略提供理论参考。方法:回顾性收集2021年1月至2023年12月在我院妇科就诊且符合纳入标准的816例不孕症患者作为研究对象,按照年龄、就诊时间以1:1比例匹配普通女性作为对照组,采用PCR-反向点杂交法对全部研究对象的宫颈脱落细胞标本进行HPV基因亚型检测,对HPV亚型感染情况进行统计分析。结果:不孕组HPV总体感染率(34.44%)、单一感染率(19.73%)、多重感染率(14.71%)及单一高危亚型感染率(16.54%)均明显高于对照组普通女性(23.77%、15.44%、8.33%、12.50%),差异均有统计学意义(χ^(2)=22.477、χ^(2)=5.179、χ^(2)=16.256、χ^(2)=5.376,均P<0.05)。单一感染高危亚型分布构成、单一感染低危亚型分布构成以及多重感染分布构成比较在不孕组与对照组之间均未显示有统计学意义(χ^(2)=13.615、P=0.627;χ^(2)=1.133、P=0.951;χ^(2)=0.295,P=0.863)。在不孕组中,≤25岁、26~30岁、31~35岁、36~40岁各个年龄组HPV总体感染率分别为:25.00%、40.36%、31.76%、31.79%,差异有统计学意义(χ^(2)=7.541,P=0.042);26~30岁、31~35岁、36~40岁3各年龄组均表现为不孕组女性HPV总体感染率明显高于对照组(40.36%vs23.97%,31.7%vs24.30%,31.79%vs21.82%),差异均有统计学意义(χ^(2)=17.508、χ^(2)=4.551、χ^(2)=4.270,均P<0.05)。结论:不孕女性的HPV总体感染率、单一感染率、多重感染率以及单一高危亚型感染率均明显高于普通女性,26~30岁不孕女性的HPV感染率最高,提示在不孕患者的日常诊疗工作中,须重视HPV的筛查。Objective:To investigate the infection and the subtype distribution of cervical human papillomavirus(HPV)in infertile women,and the differences between them and ordinary women,in order to provide rationale for developing cervical cancer screening and prevention strategies for special populations.Methods:816 infertile patients who met the inclusion criteria in the gynecology department of our hospital from January 2021 to December 2023 were retrospective collected as the study subjects.Ordinary women were matched as the control group in a 11 ratio by age and visit time.Reverse line blot hybridization was used to detect HPV gene subtypes in cervical exfoliated cell samples of all study subjects,and statistical analysis was conducted on HPV subtype infection.Results:The overall infection rate,single infection rate,multiple infection rate,and single high-risk subtype infection rate of HPV in the infertility group were significantly higher than those in the control group of ordinary women(34.44%vs 23.77%,19.73%vs 15.44%,14.71%vs 8.33%,16.54%vs 12.50%),and the differences were statistically significant(χ^(2)=22.477,χ^(2)=5.179,χ^(2)=16.256,χ^(2)=5.376,all P<0.05).There were no statistically significant differences in the distribution of single high-risk subtype infection,the distribution of single low-risk subtype infection,and the distribution of multiple infections between the infertility group and the control group(χ^(2)=13.615,P=0.627;χ^(2)=1.133,P=0.951;χ^(2)=0.295,P=0.863).In the infertility women,there were statistically significant differences in the overall HPV infection rates among the under-25 year old group(25.00%),the 26-30 year old age group(40.36%),31-35 year old age group(31.76%)and the 36-40 year old age group(31.79%)(χ^(2)=7.541,P=0.042).In the age groups of 26~30,31~35 and 36~40,the overall HPV infection rates of infertile women were significantly higher than those of the ordinary women(40.36%vs 23.97%,31.7%vs 24.30%,31.79%vs 21.82%),and the differences were statistically significant(χ^(2)=1
