摘 要:目的 探讨同轴针在超声引导下经皮肝脏占位穿刺活检中的应用价值。方法 回顾性分析在我院接受超声引导下经皮肝脏占位穿刺活检的92例患者的临床资料,根据穿刺工具的不同,分为同轴活检组(46例)和常规活检组(46例),比较两组间的平均穿刺时间、取样次数、取样满意率、穿刺成功率和并发症发生率的差异。结果 92例患者中,共有80例患者获得了确切的病理结果,整体穿刺成功率为87.0%(80/92)。同轴活检组的穿刺成功率为95.7%(44/46),显著高于常规活检组的78.3%(36/46),差异有统计学意义(P<0.05)。对于直径≤3cm的肝脏占位,同轴活检组的平均穿刺时间较短,取样次数、取样满意率和穿刺成功率均显著高于常规活检组(均P<0.05);对于直径>3cm的肝脏占位,同轴活检组的平均穿刺时间仍然较短(P<0.05),两组在取样次数、取样满意率及穿刺成功率方面无显著差异(均P>0.05)。此外,同轴活检组的并发症发生率明显低于常规活检组,差异有统计学意义(P<0.05)。结论 使用同轴针进行超声引导下肝脏占位穿刺活检术缩短了操作时间,增加了取样次数,提高了取样满意率和穿刺成功率(尤其是小病灶),同时还降低了并发症的发生率,具有安全、高效和简单易行等优势。Objective To investigate the application value of coaxial needle in ultrasound-guided percutaneous biopsy of liver lesions.Methods A retrospective analysis was conducted on the clinical data of 92 patients who underwent ultrasound-guided percutaneous liver biopsy in our hospital.The patients were divided into two groups based on the biopsy tool used:the coaxial biopsy group(n=46)and the conventional biopsy group(n=46).The differences in average puncture time,number of samples taken,sampling satisfaction rate,puncture success rate,and incidence of complications between the two groups were compared.Results Among the 92 patients,80 obtained definitive pathological results,with an overall puncture success rate of 87.0%(80/92).The puncture success rate in the coaxial biopsy group was 95.7%(44/46),significantly higher than that in the conventional biopsy group 78.3%(36/46),with statistical significance(P<0.05).For liver lesions with a diameter≤3 cm,the coaxial biopsy group had a shorter average puncture time,higher number of samples taken,higher sampling satisfaction rate,and higher puncture success rate compared to the conventional biopsy group(all P<0.05).For liver lesions with a diameter>3 cm,the coaxial biopsy group stll had a shorter average puncture time(P<0.05),but there were no significant differences between the two groups in terms of number of samples taken,sampling satisfaction rate,and puncture success rate(all P>0.05).Additionally,the incidence of complications in the coaxial biopsy group was significantly lower than that in the conventional biopsy group,with statistical significance(P<0.05).ConclusioTn he use of a coaxial needle in ultrasound-guided percutaneous biopsy of liver lesions reduces operation time,increases the number of samples taken,improves sampling satisfaction rate and puncture success rate(especially for small lesions),and reduces the incidence of complications.It is safe,efficient,and easy to perform.
