儿童肝肿瘤精准肝切除与传统肝叶切除的对比研究

摘　　要：目的探讨精准肝切除术对儿童肝肿瘤的应用价值。方法采用对比研究方法,收集2012年1月至2023年11月安徽省儿童医院40例行肝肿瘤根治性切除术患者的临床资料,将施行精准肝切除术的设为精准组(19例),施行传统肝叶切除术的设为传统组(21例),比较两组患者术前基本资料,手术时长、术中出血量、输血量,术后肝功能恢复情况,引流管留置时间及术后住院时间等。结果两组患者的一般资料比较,包括性别、手术年龄、术前肝功能及肿瘤最长径等,差异无统计学意义(P>0.05)。两组患者术中情况比较差异无统计学意义。两组患者术后情况比较,肝功能恢复情况差异无统计学意义(P>0.05),术后引流管留置时间(d)及术后住院时间(d)比较中,精准组短于传统组,两组比较差异有统计学意义(P<0.05)。结论对于儿童肝肿瘤患者,采用精准肝切除术与传统肝叶切除术均可完整切除病灶,精准肝切除术对于儿童患者可以起到减轻手术打击的目的,加速术后恢复,缩短术后引流管留置时间,缩短术后住院时间。Objective To investigate the application value of precise hepatectomy for pediatric liver tumors.Methods A comparative study was used to collect the clinical data of 40 patients who underwent sense-resection of liver tumors in Anhui Children's Hospital from January 2012 to November 2023.The patients who underwent precision hepatectomy were divided into the precision group(19 cases)and those who underwent traditional hepatectomy were divided into the traditional group(21 cases).The preoperative basic data of the two groups were compared.Operation duration,intraoperative blood loss,blood transfusion,postoperative liver function recovery,drainage tube indwelling time and postoperative hospital stay,etc.Results There was no significant difference in the general data of the two groups,including gender,age of operation,preoperative liver function and longest tumor diameter(P>0.05).There was no significant difference in intraoperative conditions between the two groups.There was no statistically significant difference in the recovery of liver function between the two groups(P>0.05).The retention time of postoperative drainage tube(d)and postoperative hospital stay(d)in the precision group were shorter than those in the traditional group,and the difference between the two groups was statistically significant(P<0.05).Conclusion For children with liver tumor,both precise hepatectomy and traditional hepatic lobectomy can completely remove the lesion,and precise hepatectomy can alleviate the surgical shock,accelerate postoperative recovery,shorten the postoperative drainage catheter retention time,and shorten the postoperative hospital stay.
