摘 要:目的检测Fascin-1蛋白及DNA错配修复蛋白(mismatch repair proteins,MMRs)在散发性结直肠腺癌中的表达,分析Fascin-1蛋白在散发性结直肠腺癌侵袭转移过程中的作用及其与微卫星不稳定(microsatellite instability,MSI)的相关性。方法检测250例散发性结直肠腺癌及癌旁正常组织中4种DNA MMRs的缺失表达及Fascin-1蛋白的表达,探讨MMRs表达情况及Fascin-1蛋白的表达与散发性结直肠腺癌临床病理特征的关系,并对Fascin-1蛋白与MMRs表达情况进行相关性分析。结果Fascin-1蛋白在癌旁正常结直肠黏膜上皮几乎不表达,在散发性结直肠腺癌中85例呈高表达,高表达率为(34%,85/250)。250例标本中,31例为MSI-H,32例为MSI-L,187例为MSS,MSI-H发生率为12.4%。Fascin-1蛋白高表达与淋巴结转移及Dukes分期相关(P<0.05);MSI-H与年龄、性别无关,而与肿瘤分化程度、淋巴结转移及Dukes分期相关(P<0.05)。MSS/MSI-L与Fascin-1蛋白呈正相关(r_(s)=0.142,P<0.05)。结论与MSI-H患者相比,MSS/MSI-L散发性结直肠腺癌患者更易出现Fascin-1蛋白高表达,提示两者可能协同参与结直肠腺癌的侵袭转移,Fascin-1蛋白有望成为MSS/MSI-L结直肠腺癌治疗的潜在新型靶点。Objective To examine the expression of Fascin-1 and mismatch repair proteins(MMRs)in colorectal adenocarcinoma and to explore the role of Fascin-1 in progression of colorectal neoplasm and its correlation with microsatelite instability(MSI)status.Methods Expressions of four MMRs and Fascin-1 were analyzed from 250 colorectal adenocarcinoma and matched non-neoplastic mucosa.MMRs and Fascin-1 expression were compared with several clinicopathologic parameters of colorectal adenocarcinoma.The correlation of microsatellite instability status and Fascin-1 expression was also analyzed in colorectal adenocarcinoma.Results Fascin-1 protein were barely expressed by the normal colorectal epithelium,but overexpressed in colorectal adenocarcinoma,and the difference between groups were statistically significant(P<0.05).In 250 samples,31 presented MSI-H,32 presented MSI-L and 187 presented MSS,the occurrence rate of MSI-H in cancers was 12.4%(31/250).High expression of Fascin-1 protein correlated significantly with lymph node metastasis and Dukes stage in colorectal adenocarcinoma(P<0.05).MSI-H correlated significantly with tumor differentiation,lymph node metastasis and Dukes stage(P<0.05).The overexpression of Fascin-1 protein positively correlated with the occurrence rate of MSS/MSI-L(r_(s)=0.142,P<0.05).Conclusion Patients with MSS/MSI-L are more likely to appear the highly expression of Fascin-1 than those with MSI-H,which suggested Fascin-1 and MSS/MSI-L may be collaboratively involved in the invasion and metastasis of coloretal adenocarcinoma.Fascin-1 may have the potential to be a newtarget spot on the treatment of MSS/MSI-L colorectal adenocarcinoma.
