摘　　要：目的 探讨抗肿瘤治疗对肾功能不全恶性肿瘤患者肾功能的影响,为临床干预提供参考依据。方法 回顾性选取2022年1月至2023年12月徐州市中心医院收治接受抗肿瘤治疗的108例肾功能不全恶性肿瘤患者临床资料,采用单因素、多因素logistic回归分析影响这些患者肾功能的因素。结果 单因素分析显示,年龄、饮酒史、是否使用肾脏保护剂、抗肿瘤治疗效果对患者的肾功能有显著影响(均P<0.05);化疗+靶向、化疗+靶向+免疫抗肿瘤方案对患者的肾功能有显著影响(均P<0.05)。多因素logistic回归分析显示,年龄≥60岁、饮酒史、抗肿瘤治疗无效是影响肾功能不全恶性肿瘤患者肾功能的独立危险因素(均P<0.05)。结论 年龄≥60岁、饮酒史、抗肿瘤治疗无效的肾功能不全恶性肿瘤患者是抗肿瘤治疗中肾功能降低的高危人群,应采取针对性措施进行干预,减轻抗肿瘤治疗对患者肾功能的影响。Objective To analyze the influencing factors of renal function reduction in malignant tumor patients with renal insufficiency receiving anti-tumor therapy.Methods The clinical data of 108 malignant tumor patients with renal insufficiency treated in Xuzhou Central Hospital from January 2022 to December 2023 were collected.The factors related to the decline of renal function during anti-tumor therapy were analyzed by univariate and multivariate logistic regression.Results Univariate analysis showed that age,history of alcohol drinking,use of kidney protective agents,efficacy of anti-tumor treatment,chemotherapy+targeting therapy,chemotherapy+targeting therapy+immune therapy had significant effects on renal function(all P<0.05).Multivariate logistic regression analysis showed that age≥60 years,history of alcohol drinking,and ineffective anti-tumor therapy were independent risk factors for renal function reduction in malignant tumor patients with renal insufficiency tumor receiving anti-tumor therapy(all P<0.05).Conclusion Old age,history of alcohol drinking,and ineffective anti-tumor therapy can independently affect renal function in malignant tumor patients with renal insufficiency receiving anti-tumor therapy.
