基于“癌毒传舍”理论探讨结直肠癌转移的机制与防治

摘　　要：结直肠癌易发生远处转移,目前以手术、全身化疗、靶向治疗、局部放疗等治疗手段为主,但不良反应较为明显。在西医治疗的基础上联合中医药可以起到减毒增效的作用,对结直肠癌转移的防治具有重要意义。《灵枢·百病始生篇》依据疾病迁延的规律首提“传舍”理论,认为虚邪中人,稽留不去,息而成积。研究认为,肠腑癌毒依托其自身流窜之特性以及血脉经络、五行生克等联属关系,主要通过血行、淋巴、经络、种植等途径发生传导播散,留舍于肝、肺、脑腑、淋巴、骨等部位。治疗上当立足于整体,辨证论治,以调理脾为要,依据肠腑癌毒传舍的不同阶段以运脾、健脾、补脾立法,随证加减。此外,还可辅以活血、化痰、祛湿、益气等治法抑制肠腑癌毒通过血行、淋巴、种植、经络的方式发生转移,并根据所“舍”之脏而引发的相关临床症状,对症施治。研究论述了“癌毒传舍”的理论内涵,并从肠腑癌毒所“传”之径、易“舍”之脏以及整体防治三方面出发,探讨该理论在结肠癌发生转移、防治过程中的潜在机制和临床价值,以期深化对结直肠癌转移机制的中西医认识,并为结直肠癌转移的临床防治提供借鉴。Colorectal cancer is prone to metastasis.At present,surgery,chemotherapy,targeted therapy,radiotherapy,and other treatment methods are mainly used,but the side effects are more obvious.On the basis of Western medicine treatment,combined with traditional Chinese medicine can play an important role in reducing toxicity and increasing efficiency,which is of great significance for the prevention and treatment of colorectal cancer metastasis.According to the law of disease changes,Lingshu·Baibing Shisheng(《灵枢·百病始生篇》)first put forward the theory of dissemination and setting down.It believesd that pathogenic factors harm people's health and stay in the body for a long time,leading to the occurrence of serious diseases.This article believes that intestinal cancer relies on its own characteristics of transfer,spreads through blood,lymph,meridians and other channels,and stays in the liver,lung,brain,lymph,bone and other body parts.The treatment should be focused on regulating the spleen,based on the holistic view and syndrome differentiation.According to the different stages of intestinal cancer virus transmission,the treatment plan should be established by activating spleen,strengthening spleen and tonifying spleen.This paper analyzed the theory of Cancer toxic dissemination and setting down from three aspects:the metastatic path of intestinal cancer,the viscera that are easy to stay,and the prevention and treatment.The potential mechanism and clinical value of this theory in the treatment of colon cancer are discussed in order to deepen the understanding of the mechanism of colorectal cancer metastasis and provide reference for clinical prevention and treatment.
