摘　　要：目的探讨乳腺癌患者血清外泌体中长链非编码RNA BC200(lncRNA BC200)的表达及临床意义。方法回顾性分析2021年1月至2024年1月连云港市第一人民医院收治的100例乳腺癌患者和50例乳腺增生患者的临床资料,以及同时间体检的健康女性30例作为对照组。对血清样本中的外泌体进行提取,观察外泌体颗粒的形态特征并检测颗粒浓度。采用RT-PCR检测3组外泌体样本的lncRNA BC200表达。计量资料的2组间比较采用独立样本t检验,多组间比较采用方差分析,两两比较采用LSD法;计数资料的组间比较采用χ^(2)检验。采用受试者操作特征曲线(ROC)分析血清外泌体中lncRNA BC200对乳腺癌的诊断价值。结果外泌体呈现典型的圆形或椭圆形,具有清晰的脂质双层膜结构;乳腺癌组、乳腺增生组和健康对照组血清外泌体中lncRNA BC200的相对表达量分别为2.76±0.91、1.15±0.42和0.69±0.18,3组比较,差异有统计学意义(F=5.087,P<0.001)。进一步两两比较发现,乳腺癌组和乳腺增生组患者血清外泌体lncRNA BC200的相对表达量高于对照组(P均<0.001),乳腺癌组高于乳腺增生组(P<0.001);临床分期晚、组织学分级高、淋巴结转移的乳腺癌患者血清外泌体中lncRNA BC200的相对表达量更高(Ⅲ~Ⅳ期比Ⅰ~Ⅱ期:t=5.342,P<0.001;2~3级比1级:t=8.804,P<0.001;淋巴结转移比不转移:t=5.516,P<0.001);血清外泌体lncRNA BC200诊断乳腺癌的ROC曲线下面积为0.819(95%CI:0.803~0.967,P<0.001),敏感度为91.73%,特异度为84.05%。结论lncRNA BC200在乳腺癌患者血清外泌体中表达上调,其表达与乳腺癌组织学分级、临床分期及淋巴结转移有关,或将成为乳腺癌临床诊断的评价指标。Objective To investigate the expression and clinical significance of long non-coding RNA BC200(lncRNA BC200)in serum exosomes of breast cancer patients.Methods A retrospective analysis was conducted on clinical data from 100 breast cancer patients and 50 patients with breast hyperplasia treated in the First People’s Hospital of Lianyungang from January 2021 to January 2024.Another 30 healthy women undergoing physical examinations during the same period served as control group.Exosomes were extracted from serum samples,and the morphological characteristics and particle concentration of the exosomes were observed.RT-PCR was used to detect the expression of lncRNA BC200 in exosome samples from the three groups.For measurement data,independent sample t test was used for comparison between two groups,ANOVA for comparison between multiple groups,and LSD for pairwise comparison.Theχ^(2) test was used for comparison of categorical data between groups.The diagnostic value of lncRNA BC200 in serum exosomes for breast cancer was analyzed using the receiver operating characteristic(ROC)curve.Results Exosomes exhibited a typical round or oval morphology,with clear lipid bilayer structures.The relative expression levels of lncRNA BC200 in serum exosomes were 2.76±0.91,1.15±0.42 and 0.69±0.18 in the breast cancer group,breast hyperplasia group and healthy control group,respectively,indicating a statistically significant difference among three groups(F=5.087,P<0.001).Further pairwise comparisons revealed that the breast cancer group and breast hyperplasia group showed significantly higher expression levels of lncRNA BC200 compared with the control group(both P<0.001),and the breast cancer group showed significantly higher expression levels of lncRNA BC200 compared with the breast hyperplasia group(P<0.001).Breast cancer patients with advanced clinical stage,higher histological grade and lymph node metastasis had significantly higher expression levels of lncRNA BC200 in serum exosomes(stageⅢ-Ⅳvs stageⅠ-Ⅱ:t=5.342,P<0
