摘　　要：内翻型踝关节扭伤过程中承受的纵向力可使胫腓骨产生错缝,腓骨向下过度运动,应力集中于内侧胫腓关节,并使相关软组织偏离正常运动轨迹,引发腓骨垂直运动机制障碍,导致胫腓骨-距骨滑车的力学联动失衡。腓骨的垂直运动机制障碍可使急性内翻型踝关节踝扭伤发展为陈旧性内翻型踝关节扭伤。阎喜换教授善用拨筋通络四步法治疗本病,以还原纵向应力、复位异常结构为核心,实现了骨正筋柔的目的。该文结合生物力学相关研究总结阎喜换教授治疗内翻型踝关节扭伤的临床经验,以期为临床提供一定参考。The longitudinal force sustained in the course of varus angle sprain will cause the tibia and fibula fissure and the excessive downward movement of the fibula.The stress concentrates on the medial tibia and fibula joints,and the related soft tissues deviates from the normal movement trajectory,resulting in the vertical movement mechanism disorder of the fibula,leading to the imbalance of the mechanical interaction between the tibiofibula and the talus trochlea.The vertical movement mechanism disorder of the fibula results in the development from acute varus ankle sprain to old varus ankle sprain.Yan Xihuan is experienced in treating this disease using the four-step method of manipulation of resetting the tendons and dredging the collaterals,in which the core is to restore the longitudinal stress and reset the abnormal structure,so as to realize the goal of solid skeleton and pliable tendons.Based on the study of biomechanics,this paper summarizes the clinical experience of Yan Xihuan in the treatment of varus ankle sprain,aiming to provide certain reference for clinical practice.
