摘　　要：目的:分析胃癌患者人表皮生长因子受体2(HER-2)表达水平的影响因素。方法:选取2021年1月—2023年12月甘南藏族自治州人民医院收治的139例胃癌患者作为研究对象,根据患者HER-2表达水平,分为低表达组(n=100)和高表达组(n=49)。比较两组一般资料、病理检查结果、影像学检查结果,分析胃癌患者HER-2表达水平的影响因素。结果:两组性别、年龄、脉管侵犯情况、肿瘤直径、肿瘤位置、浸润深度、TNM分期、淋巴结转移情况、Lauren分型比较,差异无统计学意义(P>0.05);高表达组远处转移、神经侵犯、肿瘤低分化占比高于低表达组,差异有统计学意义(P<0.05)。远处转移、神经侵犯、肿瘤低分化是胃癌患者HER-2表达的独立危险因素(P<0.05)。结论:胃癌患者HER-2表达可能与远处转移、神经侵犯、肿瘤低分化有关,临床有望通过检测HER-2水平准确评估疾病进展,制定针对性的治疗方案。Objective:To analyze the influencing factors for human epidermal growth factor receptor-2(HER-2)expression level in gastric cancer patients.Methods:A total of 139 gastric cancer patients admitted to Gannan Tibetan Autonomous Prefecture People's Hospital from January 2021 to December 2023 were selected as the study subjects.They were divided into low expression group(n=100)and high expression group(n=49)according to the HER-2 expression level.The general data,pathological examination results and imaging examination results of the two groups were compared.The influencing factors for HER-2 expression level in gastric cancer patients were analyzed.Results:There were no significant differences in gender,age,vascular invasion,tumor diameter,tumor location,invasion depth,TNM stage,lymph node metastasis,and Lauren type between the two groups(P>0.05).The proportions of distant metastasis,nerve invasion and tumor low differentiation in the high expression group were higher than those in the low expression group,and the difference was statistically significant(P<0.05).Distant metastasis,nerve invasion and tumor low differentiation were independent risk factors for HER-2 expression in gastric cancer patients(P<0.05).Conclusion:HER-2 expression in gastric cancer patients may be related to distant metastasis,nerve invasion and tumor low differentiation.It is expected that the HER-2 level can be used to accurately assess disease progression in clinical setting and formulate targeted treatment scheme.
