衍射型Tecnis多焦点人工晶状体对高度近视白内障视觉质量及视功能的影响

摘　　要：目的探讨衍射型Tecnis多焦点人工晶状体对高度近视白内障病人视觉质量及视功能的影响。方法选择2020年7月至2022年6月新乡医学院第三附属医院收治的60例高度近视白内障病人,根据简单随机分组法将其分为Restor组30例,给予衍射型Restor多焦点人工晶状体;Tecnis组30例,予以衍射型Tecnis多焦点人工晶状体。对比两组治疗前后裸眼视力、最佳矫正视力(BCNVA)、离焦曲线及主客观视觉质量及视功能指数。结果术后1周和3个月,Tecnis组裸眼近视力(UCNVA)、裸眼远视力(UCDVA)、BCNVA分别为(0.61±0.13)logMAR、(0.46±0.11)logMAR、(0.46±0.12)logMAR、(0.24±0.06)logMAR、(0.23±0.05)logMAR、(0.23±0.07)logMAR,均优于Restor组的(0.68±0.14)logMAR、(0.55±0.13)logMAR、(0.57±0.15)logMAR、(0.33±0.08)logMAR、(0.30±0.07)logMAR、(0.31±0.08)logMAR(P<0.05),最佳矫正远视力(BCDVA)与Restor组比较差异无统计学意义(P>0.05);Tecnis组在-1.50~-5.00 D的离焦水平上的视力优于Restor组(P<0.05);Tecnis组客观散射指数(OSI)、斯特列尔比值(SR)、MTF截止空间频率(MTF cut off)、不同对比度视力(OV 9%、OV 20%、OV 100%)分别为1.77±0.36、0.17±0.05、25.35±3.98、0.78±0.16、0.83±0.19、0.85±0.18,均高于Restor组的1.59±0.31、0.14±0.04、23.28±3.52、0.71±0.14、0.73±0.16、0.75±0.15(P<0.05);Tecnis组阅读脱镜率、不同距离视物满意度与Restor组比较差异无统计学意义(P>0.05)。Tecnis组视功能指数(VF-14)评分为(87.56±4.37)分,高于Restor组的(84.38±4.22)分(P<0.05)。结论衍射型Tecnis多焦点人工晶状体能够明显改善高度近视白内障病人视觉质量,提高其视功能。Objective To investigate the effects of diffractive Tecnis multi-focus intraocular lens(IOL)on visual quality and visual function in patients with high myopia cataract.Methods Sixty patients with high myopia cataract admitted in the Third Affiliated Hospital of Xinxiang Medical University from July 2020 to June 2022 were divided into two groups according to simple randomization method.One group was given diffractive Restor multi-focus intraocular lens(n=30,Restor group),and the other group was given diffractive Tecnis multi-focus intraocular lens(n=30,Tecnis group),the naked eye visual acuity,best corrected visual acuity,axial length,subjective and objective visual quality and visual function index before and after treatment were compared between the two groups.Results One week and three months after surgery,In Tecnis group,uncorrected near visual acuty(UCNVA),uncorrected distance visual acuity(UCDVA)and besat corrected near visual acuity(BCNVA)were(0.61±0.13)logMAR,(0.46±0.11)logMAR,(0.46±0.12)logMAR,(0.24±0.06)logMAR,(0.23±0.05)logMAR and(0.23±0.07)logMAR,The results were better than(0.68±0.14)logMAR,(0.55±0.13)logMAR,(0.57±0.15)logMAR,(0.33±0.08)logMAR,(0.30±0.07)logMAR,(0.31±0.08)logMAR(P<0.05),there was no significant difference between best correct distant visual acuity(BCDVA)group and Restor group(P>0.05);There was no significant difference in axial length between Tecnis group and Restor group before surgery(P>0.05).objective scattering index(OSI),strehl ratio(SR),modulation transfer function cut off frequency(MTF)cut off,different contrast visual acuity(OV 9%,OV 20%,OV 100%)were 1.77±0.36,0.17±0.05,25.35±3.98,0.78±0.16,0.83±0.19,0.85±0.18.They were higher than those in Restor group 1.59±0.31,0.14±0.04,23.28±3.52,0.71±0.14,0.73±0.16,0.75±0.15(P<0.05).There were no significant differences between Tecnis group and Restor group in reading demirrorization rate and visual satisfaction at different distances(P>0.05).The score of Visual Funtion Index 14(VF-14)in Tecnis group was(87.56±4.3
