百合地黄汤加味联合盐酸度洛西汀肠溶胶囊治疗抑郁症临床研究

摘　　要：目的:观察百合地黄汤加味联合盐酸度洛西汀肠溶胶囊治疗抑郁症的临床疗效。方法:选取82例抑郁症患者,按随机数字表法分为联合组与西药组各41例。西药组给予盐酸度洛西汀肠溶胶囊治疗,联合组在西药组基础上联合百合地黄汤加味治疗。比较2组临床疗效,比较2组治疗前后汉密尔顿抑郁量表(HAMD)评分、抑郁自评量表(SDS)评分、中医证候积分、生活质量(SF-36)评分及血清皮质醇(CORT)、丙二醛(MDA)、超氧化物歧化酶(SOD)水平的变化。结果:联合组临床疗效总有效率92.68%,西药组为70.73%,2组临床疗效比较,差异有统计学意义(P<0.05)。治疗4周、8周后,2组HAMD、SDS评分均较治疗前下降(P<0.05),联合组HAMD、SDS评分均低于同一时间段西药组(P<0.05)。治疗后,2组表情淡漠、胸闷气短、沉默寡言、抑郁寡欢中医证候积分均较治疗前下降(P<0.05),联合组上述4项中医证候积分均低于西药组(P<0.05)。治疗后,2组血清CORT、MDA水平均较治疗前下降(P<0.05),血清SOD水平均较治疗前上升(P<0.05);联合组血清CORT、MDA水平均低于西药组(P<0.05),血清SOD水平高于西药组(P<0.05)。治疗后,2组躯体疼痛、生理机能、生理职能、心理健康、情感职能、精神健康、生命活力、社会功能、总体健康SF-36评分均较治疗前升高(P<0.05),联合组上述9项评分均高于西药组(P<0.05)。治疗后,联合组并发症发生率7.32%,西药组并发症发生率12.20%,2组比较,差异无统计学意义(P>0.05)。结论:百合地黄汤加味联合盐酸度洛西汀肠溶胶囊治疗抑郁症临床疗效较好,能缓解抑郁症症状及临床应激反应,改善生活质量,安全性高。Objective:To observe the clinical effect of the combination use of modified Baihe Dihuang Decoction and Duloxetine Hydrochloride Enteric Capsules on depression.Methods:A total of 82 patients with depression were selected and divided into the combination group and the western medicine group according to the random number table method,with 41 cases in each group.The western medicine group was treated with Duloxetine Hydrochloride Enteric Capsules,and the combination group was additionally treated with modified Baihe Dihuang Decoction based on the treatment of the western medicine group.The clinical effects,and the changes of scores of Hamilton Depression Scale(HAMD),Self-Rating Depression Scale(SDS)and 36-Item Short Form Health Survey(SF-36),traditional Chinese medicine syndrome scores,and levels of serum cortisol(CORT),malondialdehyde(MDA)and superoxide dismutase(SOD)before and after treatment were compared between the two groups.Results:The total effective rate was 92.68%in the combination group and 70.73%in the western medicine group,the difference being significant(P<0.05).After four and eight weeks of treatment,the scores of HAMD and SDS in the two groups were decreased when compared with those before treatment(P<0.05),and the above two scores in the combination group were lower than those in the western medication group at the same time(P<0.05).After treatment,the traditional Chinese medicine syndrome scores of apathy,chest tightness and shortness of breath,taciturnity and depression in the two groups were lower than those before treatment(P<0.05),and the above four scores in the combination group were lower than those in the western medicine group(P<0.05).After treatment,the levels of serum CORT and MDA in the two groups were reduced when compared with those before treatment(P<0.05),and the levels of serum SOD were elevated when compared with those before treatment(P<0.05);the levels of serum CORT and MDA in the combination group were lower than those in the western medicine group(P<0.05),and the serum SOD l
