尖锐湿疣亚临床感染人乳头瘤病毒分型与预后的关系

摘　　要：目的分析尖锐湿疣亚临床感染人乳头瘤病毒(human papilloma virus,HPV)分型与预后关系。方法选取2019年1月至2021年12月江西省中西医结合医院收治的100例尖锐湿疣患者作为研究对象,根据疾病类型分为亚临床组(n=41)与典型组(n=59),检测局部疣体标本的HPV基因型,比较两组感染情况和HPV-DNA感染类型,统计亚临床感染HPV类型不同的顽固性复发情况。结果亚临床组中共检出41例类型不同的HPV-DNA,其中21例为HPV6型,占比51.22%,6例为HPV11型,占比14.63%;两组HPV-DNA感染类型比较差异无统计学意义;两组中感染高危型、低危型不同分型占比比较差异无统计学意义;与亚临床组相比,典型组三重及以上感染率略高,但组间比较差异无统计学意义;亚临床组中,6例患者复发次数≥3次,占比14.63%,其中HPV43型1例,HPV6+11型1例,HPV18型1例,HPV16型1例,为HPV 16+52型1例,HPV16+18型1例。结论尖锐湿疣亚临床感染HPV分型以HPV6型为主,多数为单一感染,且多重感染者具有较高的复发率,预后较差。Objective To analyze the relationship between human papilloma virus types and prognosis of subclinical infection of condyloma acu-minatum.Methods 100 patients with condyloma acuminata treated in Jiangxi Hospital of Integrated Traditional Chinese and Western Medicine from January 2019 to December 2021 were selected as research subjects,and they were divided into the subclinical group(n=41)and the typical group(n=59)according to disease types,HPV genotypes of local wart specimens were detected,and infection conditions and HPV-DNA infection types be-tween the two groups were compared,the recurrence of subclinical HPV types was analyzed.Results A total of 41 different types of HPV-DNA were detected in the subclinical group,of which 21 cases were HPV type 6,accounting for 51.22%,and 6 cases were HPV type 11,accounting for 14.63%.There was no significant difference in HPV-DNA infection types between the two groups.There was no significant difference in the propor-tion of high-risk and low-risk infection types between the two groups.Compared with subclinical group,the triple or above infection rate was slightly higher in typical group,but there was no statistical significance between the two groups.In the subclinical group,6 patients had≥3 recurrence times,accounting for 14.63%,including 1 case of HPV 43,1 case of HPV 6+11,1 case of HPV 18,1 case of HPV 16,1 case of HPV 16+52,and 1 case of HPV 16+18.Conclusion HPV type 6 is the main subclinical infection of condyloma acuminata,most of them are single infection,and multiple in-fection has a high recurrence rate and poor prognosis.
