双心境稳定剂联合安神汤在双相情感障碍躁狂发作中的应用研究

摘　　要：目的观察双心境稳定剂联合安神汤在双相情感障碍(BD)躁狂发作治疗中的应用效果。方法选取2022年6月至2023年6月收治且满足入组标准的BD躁狂发作患者100例为研究对象,基于患者一般资料具有可比性原则下将纳入研究对象随机分为对照组、观察组。对照组50例给予双心境稳定剂碳酸锂片+丙戊酸钠缓释片治疗,观察组在对照组基础上联合安神汤治疗。评估与比较两组治疗效果、中医证候积分变化,检测并观察患者治疗前后血清多巴胺(DA)、五羟色胺(5-HT)、皮质醇(Cor)、促肾上腺皮质激素(ACTH)等血生化水平变化情况,以治疗副反应量表(TESS)评价不良反应状况。结果治疗6周后,观察组治疗总有效率(90.00%)高于对照组(74.00%)(P<0.05),观察组患者中医症候总积分低于对照组(P<0.05),观察组5-HT水平高于对照组(P<0.05),DA、Cor、ACTH水平低于对照组(P<0.05),而观察组与对照组治疗1、6周后的TESS评分比较差异无统计学意义(P>0.05)。结论双心境稳定剂联合安神汤在BD狂躁发作治疗中,可进一步改善患者临床症状,调节神经系统内环境平衡,继而提高疗效,是一种有效安全的治疗方案。Objective To explore the effect of Anshen Decoction combined with dual mood stabilizers in the treatment of bipolar disorder manic episode(BPD-ME).Methods One hundred eligible patients with BPD-ME admitted to the psychiatric department of the hospital from June 2022 to June 2023 were enrolled.They were divided into a control group and an observation group based on the principle of comparability.All patients were managed by dual mood stabilizers(lithium carbonate tablets+sodium valproate sustained-release tablets),and those in observation group were additionally treated with Anshen Decoction.A 6-week of treatment were performed,aiming to compare curative effect,traditional Chinese medicine(TCM)symptom score,serum dopamine(DA),5-hydroxy tryptamine(5-HT),cortisol(Cor),adrenocorticotropic hormone(ACTH).The adverse reactions were assessed by Treatment Emergent Symptom Scale(TESS).Results At week 6 of treatment,the overall effective rate in the observation group was significantly higher than that in the control group(90%vs 74%,P<0.05).TCM symptom scores decreased notably in the observation group compared with the control group(P<0.05).At week 6 of treatment,significantly higher 5-HT level,and lower DA,Cor and ACTH levels were found in observation group than those of control group(all P<0.05).The 1-week and 6-week TESS scoresin the observation group werecompared to the control group(P>0.05).Conclusion The combination of Anshen Decoction and dual mood stabilizers for patients with BPD-ME can further improve clinical symptoms,regulate the balance in the internal environment of the nervous system,and enhance therapeutic effects.It is effective and safe.
