清头定痛汤联合刺络放血疗法对风痰瘀阻型偏头痛患者的疗效及对脑电图和血液流变学的影响

摘　　要：目的探讨清头定痛汤联合刺络放血疗法对风痰瘀阻型偏头痛患者的疗效。方法选取2021年7月至2022年6月医院收治的80例风痰瘀阻型偏头痛患者,按照随机数字表法分为对照组40例和研究组40例,对照组采用常规西药治疗,研究组采用清头定痛汤联合刺络放血疗法治疗。比较2组治疗前及治疗后1个月头痛积分、脑电图及血液流变学指标变化,并观察2组临床疗效及不良反应发生情况。结果治疗后1个月2组头痛发作次数、头痛程度、头痛持续时间、伴随症状及总积分均低于治疗前(P<0.05),且研究组各项头痛积分均低于对照组(P<0.05);研究组θ波增多率、δ波增多率及α波频率变慢率均高于对照组(P<0.05);治疗后1个月研究组血浆黏度、全血黏度(低切)、全血黏度(高切)及纤维蛋白原均低于治疗前(P<0.05),且研究组血液流变学指标均低于对照组(P<0.05);而对照组治疗前后血液流变学指标比较,差异无统计学意义(P>0.05);研究组治疗总有效率高于对照组(P<0.05);2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论清头定痛汤联合刺络放血疗法治疗风痰瘀阻型偏头痛疗效良好,能有效改善患者脑电图变化及血液流变学,安全性较好。Objective To explore the efficacy of QingtouDingtong Decoction combined with blood-letting puncture therapy for patients with migraine of phlegm wind resistance type.Methods Eighty patients with migraine(phlegm wind resistance type)admitted to the hospital from July 2021 to June 2022 were enrolled,and they were randomly assigned into a control group(n=40)and a study group(n=40),in which they were given conventional Western medicine and QingtouDingtong Tang combined with pricking and blood-letting therapy,respectively.The aim was to compare migraine score,electroencephalogram(EEG),hemorheology indexes on day 0,and on month 1.The curative effect and adverse reactions were assessed.Results The dimension score and total score of migraine(including headache attack frequency,degree of headache,headache duration,associated symptoms)in both groups on month 1 were significantly lower than those on day 0,which were significantly lower in the study group than in the control group(P<0.05).The increased theta/delta activity,slowed alpha wave frequency were significantly higher in the study group than in the control group(P<0.05).The plasma viscosity(PV),whole blood viscosity(WBV)(low shear and high shear),fibrinogen in the study group on month 1 were significantly lower than those on day 0(P<0.05),and hemorheology indexes in the study group were significantly lower than those of the control group(P<0.05).None of the hemorheology indexes differed significantly in the control group before and after treatment(P>0.05).The overall effective rate in the study group was significantly higher than that in the control group(P<0.05).The overall incidence of adverse reactions between the two groups was comparable(P>0.05).Conclusion For patients with migraine(phlegm wind resistance type),QingtouDingtong Decoction combined with blood-letting puncture therapy has good therapeutic effect,which can effectively improve indexes of both EEG and hemorheology,and it has a good safety.
