加减升陷汤治疗非小细胞肺癌合并癌因性疲乏患者疗效及对促炎性相关因子的影响

摘　　要：目的探讨加减升陷汤治疗非小细胞肺癌合并癌因性疲乏患者疗效及对促炎性相关因子的影响。方法选取2023年5月-2024年5月中国中医科学院广安门医院南区收治的非小细胞肺癌合并癌因性疲乏患者80例,根据随机数字表法分为观察组和对照组,每组40例,对照组采用化疗治疗,观察组患者在对照组基础上加用加减升陷汤治疗,两组患者均持续治疗2个月。比较两组患者的临床疗效,并对两组患者治疗前后Piper疲乏量表评分、促炎性细胞因子水平进行比较,观察两组患者不良反应发生情况。结果观察组患者癌因性疲乏治疗总有效率(95.00%)明显高于对照组(72.50%)(χ^(2)=7.440,P<0.05)。治疗前两组患者Piper疲乏量表评分、促炎性细胞因子水平无明显差异(P>0.05);治疗后观察组患者认知评分、躯体评分、情感评分和行为评分改善程度均优于对照组(P<0.05)。治疗后观察组患者白细胞介素-6(Interleukin 6,IL-6)、肿瘤坏死因子(Tumor Necrosis Factor-α,TNF-α)和血清C反应蛋白(C-reactive protein,CRP)水平均明显低于对照组(P<0.05)。观察组患者不良反应发生率(7.50%)明显低于对照组(30.00%),差异有统计学意义(χ^(2)=6.646,P<0.05)。结论加减升陷汤治疗非小细胞肺癌合并癌因性疲乏患者有一定疗效,可明显改善患者疲乏状态,降低炎症反应,减轻不良反应。Objective To explore the therapeutic efficacy of Jiajian Shengxian Decoction on cancer-related fatigue in patients with non-small cell lung cancer after chemotherapy and its influence on pro-inflammatory factors.Methods A total of 80 non-small cell lung cancer patients with cancer-related fatigue admitted to Guang’anmen Hospital South Campus,China Academy of Chinese Medical Sciences from May 2023 to May 2024 were randomly divided into an observation group and a control group according to the random number table method,with 40 patients in each group.The control group was treated with chemotherapy,and the observation group was treated with Jiajian Shengxian Decoction on the basis of chemotherapy.Both groups were treated for two months.The clinical efficacy of the two groups was compared,the Piper fatigue scale scores and pro-inflammatory cytokine levels of the two groups were compared before and after treatment,and the occurrence of adverse reactions was observed in the two groups.Results The total effective rate of cancer-related fatigue in the observation group(95.00%)was significantly higher than that in the control group(72.50%)(χ^(2)=7.440,P<0.05).There were no significant differences in Piper fatigue scale scores and pro-inflammatory cytokine levels between the two groups before treatment(P>0.05).After treatment,the improvement of the cognitive score,physical score,emotional score,and behavioral score in the observation group were significantly superior to the control group(P<0.05).After treatment,the levels of Interleukin 6(IL-6),Tumor Necrosis Factor-α(TNF-α),and C-reactive protein(CRP)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group(7.50%)was significantly lower than that in the control group(30.00%),and the difference was statistically significant(χ^(2)=6.646,P<0.05).Conclusion Jiajian Shengxian Decoction has a significant therapeutic effect on cancer-related fatigue in patients with non-small cell l
