健脾解毒方联合西医标准治疗晚期肠癌的临床疗效观察

摘　　要：目的观察健脾解毒方联合化疗治疗脾虚型晚期肠癌的临床疗效及安全性。方法将病理确诊的晚期结直肠癌患者63例,用随机数字表法分为治疗组(n=31)和对照组(n=32)。对照组给予西医一二线标准化疗,治疗组在常规化疗的基础上加用健脾解毒方,治疗6周期。比较治疗前后两组患者中医证候疗效、中医证候积分、体力状况评分、近期临床疗效、化疗周期、毒副反应。结果①对照组中医证候总疗效有效率为56.7%,治疗组为86.7%,治疗组疗效优于对照组(P<0.05)。②治疗前后比较,对照组和治疗组均可改善患者中医证候(P<0.05),治疗组对中医证候的改善明显优于对照组(P<0.05)。③治疗前后组内比较,治疗组可以改善患者体力状况评分(P<0.05)。④治疗组患者血液毒性、胃肠道毒性发生率均低于对照组(P<0.05)。⑤治疗组患者在实体瘤近期临床疗效评价及完成足周期化疗上,具有一定优势。结论健脾解毒方联合西医标准治疗晚期肠癌患者可改善患者中医证候,提高生活质量,减轻患者化疗毒副反应。Objective To observe the clinical efficacy and safety of Jianpi Jiedu Decoction combined with chemotherapy in the treatment of advanced colorectal cancer with spleen deficiency syndrome.Methods 63 patients with advanced colorectal cancer were divided into the treatment group(n=31)and the control group(n=32)using the random number table method.The control group was treated with first-and second-line standard chemotherapy of western medicine,and the treatment group was treated with Jianpi Jiedu Decoction on the basis of conventional chemotherapy for 6 cycles.The efficacy of traditional Chinese medicine(TCM)syndrome,TCM syndrome score,physical status score,recent clinical efficacy,chemotherapy cycle,toxic and side effect were compared between the two groups before and after treatment.Results①The total effective rate was 56.7%in the control group and 86.7%in the treatment group.The curative effect of the treatment group was better than that of the control group(P<0.05).②Compared before and after treatment,both the control group and the treatment group could improve TCM syndrome(P<0.05),but the improvement of TCM syndrome in the treatment group was significantly superior to the control group(P<0.05).③Compared before and after treatment,the score of physical status in the treatment group could be improved(P<0.05).④The incidence of blood toxicity and gastrointestinal toxicity in the treatment group was lower than that in the control group(P<0.05).⑤The treatment group patients had certain advantages in recent clinical efficacy evaluation of solid tumors and completion of full cycle chemotherapy.Conclusion The combination of Jianpi Jiedu Decoction and standard western medicine treatment can improve the TCM syndrome,enhance the quality of life,and alleviate toxic and side effect of chemotherapy in patients with advanced colorectal cancer.
