基于扎根理论的儿童癌症医疗服务能力的制约因素研究——以青海省为例

摘　　要：目的:以青海省为例,探究儿童癌症医疗服务能力的制约因素,为进一步改善青海省等欠发达地区的儿童癌症医疗服务能力提供政策依据。方法:采用目的抽样,于2023年12月,对青海省的19名儿童癌症患者照顾者和17名医务人员进行半结构式访谈。采用Strauss程序化扎根理论的编码思路,并运用MAXQDA2020定性分析软件对访谈资料进行分析。结果:共提炼得到26个初始概念和13个范畴,并构建出儿童癌症医疗服务能力的制约因素模型。结论:儿童癌症医疗服务能力的制约因素是复杂多元的,包括社会环境约束、政策制度问题、病源严重流失以及人力资源困境等四个维度。因此,应逐步探索“慈善+医疗”模式,切实加强远程会诊中心、儿童癌症培训体系建设,从而改善青海省等欠发达地区的儿童癌症医疗服务能力。Objective To explore the restrictive factors of pediatric oncology healthcare service capacity in Qinghai Province,and provide policy recommendations to improve service capacity in underdeveloped regions.Methods 19 caregivers of pediatric cancer patients and 17 healthcare professionals were chosen from Qinghai Province by using purposive sampling,and conducted semi-structured interviews in December 2023.The Strauss procedural coding approach of grounded theory and MAXQDA2020 qualitative analysis software were employed to analyze the interview data.Results A total of 26 initial concepts and 13 categories were extracted to construct a model of the restrictive factors on pediatric oncology healthcare service capacity.Conclusion The restrictive factors are complex and multifaceted,encompassing four dimensions of social environment constraints,policy limitations,severe patient outflow,and human resource challenges.Therefore,it is imperative to gradually explore a"charity and medicine"model,effectively strengthen teleconsultation centers and pediatric oncology training system construction to improve the healthcare service capacity for pediatric oncology in underdeveloped regions like Qinghai Province.
