重复经颅磁刺激治疗臂丛神经损伤的疗效观察

摘　　要：目的:评估重复经颅磁刺激(rTMS)治疗在臂丛神经损伤患者中的疗效。方法:选取从2016年1月到2020年12月我院收治的106例臂丛神经病患者,经过系统的回顾性研究,将其随机划分为对照组58例,观察组48例,对照组接受传统的针灸、电疗以及营养神经等治疗,观察组增加TMS治疗,每天一次,每周五天,两天休息,总计接受了50-150次治疗,最终经过6-10个月的跟踪调查,通过Mallett肩关节、Gilbert肘关节和Raimondi手关节的功能评估,视觉模拟评分评估疼痛情况。结果:观察组的Mallett肩功能、Gilbert肘功能、Raimondi手功能评分分别达到86.21%、91.38%和77.59%,明显高于对照组,经比较差异具有统计学意义(P<0.05)。结论:TMS辅助传统康复治疗可有效提高臂丛神经肢体功能恢复、减轻疼痛,值得临床广泛推广。Objective:To evaluate the efficacy of repetitive transcranial magnetic stimulation(rTMS)in patients with brachial plexus injury.Methods:A total of 106 patients with brachial plexus neuropathy admitted to our hospital from January 2016 to December 2020 were randomly divided into a control group of 58 patients and an observation group of 48 patients.The control group received traditional acupuncture and moxibustion,electrotherapy,and nutritional nerve treatment.The observation group received TMS treatment once a day,five days a week,and two days of rest.A total of 50-150 times of treatment were received.Finally,after 6-10 months of follow-up investigation.The pain was assessed by Mallett Shoulder joint,Gilbert elbow joint and Raimondi hand joint function assessment and visual simulation score.Result:The Mallett shoulder function,Gilbert elbow function,and Raimondi hand function scores of the observation group were 86.21%,91.38%,and 77.59%,respectively,which were significantly higher than those of the control group.After comparison,the differences were statistically significant(P<0.05).Conclusion:TMS assisted traditional rehabilitation therapy can effectively improve the recovery of brachial plexus limb function and alleviate pain,and is worthy of widespread clinical promotion.
