摘  要:目的 探讨侵袭性前列腺癌MRI影像表现与血清微小核糖核酸27a(miR-27a)、α-甲酰基辅酶A消旋酶(AMACR)水平的关系及其预后分析.方法 112例前列腺癌患者根据术后病理分级结果将前列腺癌分为中低危组(n=61)和高危组(n=51).所有患者术前1周内均完善MRI及血清miR-27a、AMACR检查.采用Spearman秩相关分析2组患者MRI影像表现与血清miR-27a和AMACR水平的相关性.所有患者术后均随访24个月以上,分析2组患者预后结局.结果 与中低危组相比,高危组的表观扩散系数(ADC)值较低,容量转移常数(K^(trans))和速率常数(K_(ep))较高(P<0.05).与中低危组相比,高危组的血清miR-27a和AMACR水平较高(P<0.05).经Spearman秩相关分析显示,ADC值与血清miR-27a和AMACR水平呈负相关(P<0.05),K_(ep)、K^(trans)与血清miR-27a和AMACR水平呈正相关(P<0.05).高危组出现复发和转移的患者明显多于中低危组(P<0.05).结论 MRI影像表现及血清miR-27a、AMACR水平可用于评估前列腺癌侵袭性及预后.Objective To investigate the relationship between MRI imaging findings of invasive prostate cancer and serum miR-27a and alpha-methylacyl-CoA racemase(AMACR)levels and its prognostic analysis.Methods A total of 112 patients with prostate cancer were divided into middle-low risk group(n=61)and high-risk group(n=51)according to the results of postoperative pathological grading.All patients underwent MRI and serum miR-27a and AMACR examinations within 1 week before surgery.Spearman rank correlation was used to analyze the correlation between MRI imaging findings and serum miR-27a and AMACR levels in the two groups.All patients were followed up for more than 24 months after surgery,and the prognosis of the two groups was analyzed.Results Compared with the middle-low risk group,the apparent dfusion coefficient(ADC)value in the high-risk group was lower,and the K^(trans) and K_(ep) were higher(P<0.05).Compared with the middle-low risk group,the serum miR-27a and AMACR levels in the high-risk group were higher(P<0.05).Spearman rank correlation analysis showed that ADC value was negatively correlated with serum miR-27a and AMACR levels(P<0.05),and K_(ep) and K^(trans) were positively correlated with serum miR-27a and AMACR levels(P<0.05).The patients with recurrence and metastasis in the high-risk group were significantly more than those in the middle-low risk group(P<0.05).Conclusion 1MRI imaging findings and serum miR-27a and AMACR levels can be used to evaluate the invasiveness and prognosis of prostate cancer.
