加味逍遥散联合碳酸锂治疗双相情感障碍抑郁发作患者的效果

摘　　要：目的:观察加味逍遥散联合碳酸锂治疗双相情感障碍(BD)抑郁发作患者的效果。方法:选取2021年12月至2023年12月余干县精神病医院收治的124例BD抑郁发作患者进行前瞻性研究,按照随机数字表法分为研究组与对照组各62例。对照组采用碳酸锂片治疗,研究组在对照组基础上联合加味逍遥散治疗,比较两组汉密尔顿抑郁量表(HAMD)评分、正性负性情绪自评量表(PANAS)评分、伯克利情绪表达量表(BEQ)评分、血清氧化应激指标[过氧化氢酶(CAT)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)]水平、血清炎性因子[白细胞介素-6(IL-6)、巨噬细胞移动抑制因子(MIF)、白细胞介素-1β(IL-1β)]水平以及不良反应发生率。结果:治疗后,研究组PANAS正性情绪评分、BEQ评分以及血清CAT、GSH-Px、SOD水平高于对照组,HAMD评分、PANAS负性情绪评分以及血清IL-6、IL-1β、MIF等血清炎性因子水平低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:加味逍遥散联合碳酸锂治疗BD抑郁发作患者可提高PANAS正性情绪评分、BEQ评分,改善氧化应激指标水平,以及降低HAMD评分、PANAS负性情绪评分和血清炎性因子水平,效果优于单纯碳酸锂治疗。Objective:To observe effects of Jiawei Xiaoyao powder combined with Lithium carbonate in treatment of patients with bipolar disorder(BD)depressive episodes.Methods:A prospective study was conducted on 124 patients with BD depressive episodes admitted to Yugan County Psychiatric Hospital from December 2021 to December 2023.According to the random number table method,they were divided into study group and control group,62 cases in each group.The control group was treated with Lithium carbonate tablets,while the study group was treated with Jiawei Xiaoyao powder on the basis of that of the control group.The Hamilton depression scale(HAMD)score,the positive and negative emotion symptom scale(PANAS)score,the Berkeley emotional questionnaire(BEQ)score,the serum oxidative stress indexes[catalase(CAT),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)]levels,the serum inflammatory factors[interleukin-6(IL-6),macrophage migration inhibitory factor(MIF),interleukin-1β(IL-1β)]levels,and the incidence of adverse reactions were compared between the two groups.Results:After the treatment,the PANAS positive emotion score and the BEQ score and the levels of serum CAT,GSH-Px and SOD in the study group were higher than those in the control group;the HAMD score,the PANAS negative emotion score and the levels of serum inflammatory factors such as IL-6,IL-1βand MIF in the study group were lower than those in the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Jiawei Xiaoyao powder combined with Lithium carbonate can improve the PANAS positive emotion scores and the BEQ scores,improve the oxidative stress index levels,and reduce the HAMD scores,the PANAS negative emotion scores and the serum inflammatory factor levels in the patients with BD depressive episodes.Moreover,it is superior to simple Lithium carbonate treatment.
