摘　　要：目的分析经腹腔镜子宫肌瘤剥除术对子宫肌瘤患者盆底功能及生育功能的影响。方法选取我院2021年1月-2023年3月收治的64例子宫肌瘤患者,按照随机数字表法将其分为观察组和对照组,每组32例。对照组行开腹子宫肌瘤剥除术,观察组行经腹腔镜子宫肌瘤剥除术,比较两组手术恢复情况、并发症发生情况、生活质量、盆底功能障碍发生率以及生育功能。结果观察组手术用时、术后首次下床时间、术后首次肛门排气时间、住院时间均短于对照组,术中出血量少于对照组(P<0.05);观察组术后并发症发生率及复发率均低于对照组(P<0.05);观察组术后生活质量各维度评分高于对照组(P<0.05);观察组盆底功能障碍发生率低于对照组(P<0.05);观察组妊娠率高于对照组,流产率低于对照组(P<0.05)。结论经腹腔镜子宫肌瘤剥除术治疗子宫肌瘤的临床效果理想,患者恢复较快,能够有效降低术后并发症发生率以及复发率,且可提升患者生活质量、盆底功能及生育功能。Objective To analyze the effect of laparoscopic myomectomy on pelvic floor function and reproductive function in patients with uterine fibroids.Methods A total of 64 patients with uterine fibroids admitted to our hospital from January 2021 to March 2023 were selected and divided into observation group and control group according to the random number table method,with 32 patients in each group.The control group underwent open myomectomy,and the observation group underwent laparoscopic myomectomy.The surgical recovery,complications,quality of life,incidence of pelvic floor dysfunction and reproductive function were compared between the two groups.Results The operation time,the first time of getting out of bed after operation,the first time of anal exhaust after operation and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group(P<0.05).The incidence of postoperative complications and recurrence rate in the observation group were lower than those in the control group(P<0.05).The scores of each dimension of quality of life after surgery in the observation group were higher than those in the control group(P<0.05).The incidence of pelvic floor dysfunction in the observation group was lower than that in the control group(P<0.05).The pregnancy rate of the observation group was higher than that of the control group,and the abortion rate was lower than that of the control group(P<0.05).Conclusion The clinical effect of laparoscopic myomectomy in the treatment of uterine fibroids is ideal,and the patients recover quickly.Meanwhile,it can effectively reduce the incidence of postoperative complications and recurrence rate,and can improve the quality of life,pelvic floor function and reproductive function of patients.
