摘 要:目的:分析授权教育联合路径化健康教育在结肠造口术患者主要照顾者中的应用效果.方法:采用便利抽样方法选取2023年1—8月徐州医科大学附属医院收治的大肠癌行结肠造口术患者的主要照顾者100名,采用随机数字表法将其分为干预组和对照组两组各50名.干预组采用授权教育联合路径化健康教育干预,对照组实施常规健康教育干预.比较两组主要照顾者干预前、干预后立即和干预后1个月照顾负担、照顾能力,以及干预后1个月患者造口并发症发生情况.结果:干预后立即及干预后1个月,干预组主要照顾者照顾负担总分及责任负担、个人负担评分,照顾能力总分及各维度评分(适应照顾角色、应变及提供帮助、处理个人情绪需要、评估家人及社区资源、调整个人及照顾需求)均低于对照组,差异有统计学意义(P<0.05).干预后1个月,干预组患者造口并发症发生率低于对照组,差异有统计学意义(P<0.05).结论:授权教育联合路径化健康教育可有效减轻结肠造口术患者主要照顾者照顾负担,提升其照护能力,并降低患者造口并发症发生率.Objective To analyze the effect of empowerment education combined with path-based health education on the primary caregivers of colostomy patients.Methods A total of 100 main caregivers of colorectal cancer patients undergoing colostomy in the Affiliated Hospital of Xuzhou Medical University from January to August 2023 were selected by convenience sampling method,and were divided into intervention group and control group with 50 cases in each group by random number table method.The intervention group received empowering education combined with path-based health education,and the control group received routine health education.The care burden and care capacity of the two groups of primary caregivers were compared before intervention,immediately after intervention and 1 month after intervention,and the incidence of ostomy complications was compared between the two groups 1 month after intervention.Results Immediately after intervention and one month after intervention,the total score of caregiving burden,responsibility burden and personal burden score of the main caregivers in the intervention group,and the total score of caregiving ability and scores in various dimensions(adapting to caregiving roles,responding to and providing assistance,dealing with personal emotional needs,assessing family and community resources,adjusting personal and caregiving needs)were lower than those in the control group,the difference was statistically significant(P<0.05).One month after the intervention,the incidence of ostomy complications in the intervention group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions Empowering education combined with path-based health education can effectively reduce the care burden of primary caregivers of colostomy patients,improve their caring ability,and reduce the incidence of stomy complications.
