翼状胬肉术中应用重组牛碱性成纤维细胞生长因子眼用凝胶对眼表的保护作用研究

摘　　要：目的:观察翼状胬肉术中应用重组牛碱性成纤维细胞生长因子(rb-bFGF)眼用凝胶对眼表的保护作用.方法:选取2019年1月—2021年12月于青岛大学附属威海市立第二医院行翼状胬肉切除加自体角膜缘干细胞移植术的患者60例(60眼),根据治疗方案不同将其分为观察组和对照组各30例.观察组术中应用rb-bFGF保护非手术区眼表,对照组术中使用0.9%氯化钠注射液棉片保持非手术区眼表湿润.比较两组角膜刺激症状评分、角膜上皮完全愈合时间、泪膜破裂时间(BUT)和泪液分泌试验(SIt)结果.结果:术后1、3 d,观察组角膜刺激症状评分均低于对照组;术后1个月,观察组BUT长于对照组,SIt优于对照组,差异有统计学意义(P<0.05).术后,观察组角膜上皮完全愈合时间短于对照组,差异有统计学意义(P<0.05).结论:翼状胬肉术中应用rb-bFGF眼用凝胶可以有效保护患者眼表组织,减轻患者角膜刺激症状,有利于眼表功能恢复和稳定泪膜.Objective To observe the protective effect of recombinant bovine basic fibroblast growth factor(rb-bFGF)ophthalmic gel on ocular surface during pterygium operation.Methods A total of 60 patients(60 eyes)who underwent pterygium excision plus autolimbal stem cell transplantation in Weihai Municipal Second Hospital Affiliate to Qingdao University from January 2019 to December 2021 were selected,and were divided into observation group and control group with 30 cases in each group according to different treatment plans.In the observation group,rb-bFGF was used to protect the ocular surface in the non-operative area,while in the control group,0.9%sodium chloride injection cotton tablets were used to keep the ocular surface moist in the non-operative area.The results of corneal stimulation symptom score,complete healing time of corneal epithelium,breakup time of tear film(BUT)and SchirmerⅠtest(SⅠt)were compared between the two groups.Results On 1,3 days after operation,corneal irritation symptom score in observation group was lower than that in control group;One month after operation,the observation group BUT longer than the control group,SⅠt was better than the control group,the difference was statistically significant(P<0.05).After operation,the complete healing time of corneal epithelium in observation group was shorter than that in control group,and the difference was statistically significant(P<0.05).Conclusions The application of rb-bFGF ophthalmic gel in pterygium operation can effectively protect the ocular surface tissue,reduce the symptoms of corneal irritation,and help to restore ocular surface function and stabilize tear film.
