摘　　要：目的:研究重复经颅磁刺激(rTMS)治疗帕金森的临床疗效。方法:选取105例帕金森病患者作为研究对象,依据治疗方式不同将患者分为试验组(常规药物治疗+rTMS治疗,n=53)与对照组(常规药物治疗,n=52)。分析并比较两组患者的临床疗效、认知功能[简易智能状态量表评分(MMSE)、蒙特利尔认知评估表(MoCA)]、步态平衡指标[Berg平衡量表评分(BBS)、转身角速度、步速、步长]、基质金属蛋白酶3(MMP-3)、白细胞介素1β(IL-1β)及IL-6水平变化。结果:试验组临床总疗效率比对照组更高(92.45%vs.76.92%,P<0.05);两组治疗后MMSE、MoCA评分均上升(P<0.05),且试验组高于对照组(P<0.05);两组治疗后BBS评分、转身角速度、步速、步长均上升(P<0.05),且试验组高于对照组(P<0.05);治疗后,试验组MMP-3、IL-1β、IL-6水平均低于对照组(P<0.05)。结论:rTMS治疗帕金森可提升的临床疗效,有效改善患者认知功能、步态平衡指标和调节MMP-3、IL-1β及IL-6水平。Objective:To study the clinical effect of repetitive transcranial magnetic stimulation(rTMS)in the treatment of Parkinson's disease.Methods:A retrospective study of 105 patients with Parkinson's disease was conducted,and they were divided into experimental group(n=53,conventional drug therapy+rTMS therapy)and control group(n=52,conventional drug therapy)according to different treatment methods.The clinical efficacy,cognitive function[Mini-mental State Examination(MMSE),Montreal Cognitive Assessment(MoCA)],gait balance indexes[Berg Balance Scale(BBS),turn angle speed,step speed,step length],changes of matrix metalloproteinase-3(MMP-3),interleukin-1β(IL-1β)and interleukin-6(IL-6)levels were analysed and compared between the two groups.Results:The total clinical efficacy rate of experimental group was higher than that of control group(92.45%vs.76.92%,P<0.05).The MMSE and MoCA scores of both groups increased after treatment(P<0.05),and the experimental group was higher than the control group(P<0.05).BBS score,turn angle speed,step speed and step length increased in both groups after treatment(P<0.05),and the experimental group was higher than the control group(P<0.05).After treatment,the levels of MMP-3,IL-1βand IL-6 in experimental groups were lower than those in control group(P<0.05).Conclusion:rTMS in the treatment of Parkinson's disease can improve the clinical efficacy,effectively improve the cognitive function of patients,gait balance index,and regulate the levels of MMP-3,IL-1βand IL-6.
