摘 要:目的探讨血管内皮生长因子(VEGF)、水通道蛋白1(AQP1)表达对视网膜中央静脉阻塞术后继发黄斑水肿的早期预测价值。方法选取我院2019年4月至2021年4月收治的220例(220眼)行玻璃体腔内注射治疗的CRVO患者,术后随访6个月观察患者是否出现黄斑水肿,其中继发黄斑水肿者(继发组)120例,未继发黄斑水肿者(未继发组)100例。比较两组患者一般资料、房水VEGF、AQP1水平、不同严重程度黄斑水肿VEGF、AQP1水平,分析CRVO继发黄斑水肿因素及VEGF、AQP1对CRVO继发黄斑水肿的预测价值并绘制ROC曲线。结果继发组肿瘤坏死因子α、白介素-6、VEGF水平高于未继发组,AQP1低于未继发组(P<0.05);存在少量上视网膜内高反射点(HRDs)的肿瘤坏死因子α、白介素-6、VEGF水平低于中等量者和大量者,AQP1水平高于中等量者和大量者(P<0.05);肿瘤坏死因子α、白介素-6、VEGF、AQP1是CRVO继发黄斑水肿的独立危险因素(P<0.05);肿瘤坏死因子α、白介素-6、VEGF、AQP1联合检测预测CRVO继发黄斑水肿的AUC值高于单项检测(P<0.05)。结论VEGF在CRVO继发黄斑水肿患者中呈高表达,AQP1呈低表达,其与患者黄斑水肿严重程度密切相关,二者联合检测预测CRVO术后继发黄斑水肿的价值较高。Objective To explore the early predictive value of vascular endothelial growth factor(VEGF)and aquaporin 1(AQP1)expression in retinal tissue for macular edema after central retinal vein occlusion.Methods 220 patients(220 eyes)with CRVO treated by intravitreal injection in our hospital from April 2019 to April 2021 were selected.The patients were followed up for 6 months to observe whether they had macular edema,including 120 patients with secondary macular edema(secondary group)and 100 patients without secondary macular edema(non secondary group)..Compare the general data and the levels of VEGF and AQP1 in aqueous humor between the two groups,compare the levels of VEGF and AQP1 in macular edema of different severity,analyze the factors affecting the secondary macular edema of CRVO,analyze the predictive value of VEGF and AQP1 alone and in combination for the secondary macular edema of CRVO,and draw the ROC curve.Result The levels of tumor necrosis factor-α,interleukin-6 and VEGF in the secondary group were higher than those in the non secondary group,and AQP1 was lower than that in the non secondary group(P<0.05);There is a small amount of tumor necrosis factor in the high reflection points(HRDs)in the upper retinaα、The levels of IL-6 and VEGF were lower than those in the middle and large groups(P<0.05),and the levels of AQP1 with a small amount of HRDs were higher than those in the middle and large groups(P<0.05);Logistic regression analysis showed that tumor necrosis factorα、The levels of interleukin-6,VEGF and AQP1 were independent risk factors for macular edema secondary to CRVO(P<0.05);The AUC value of combined detection of VEGF and AQP1 in predicting macular edema secondary to CRVO was higher than that of single detection(P<0.05).Conclusion VEGF was highly expressed in patients with CRVO secondary macular edema,and AQP1 was low expressed,which was closely related to the severity of macular edema.The combined detection of VEGF and AQP1 had high value in predicting after central CRVO secondary macular
