耳穴磁疗法联合腹横肌平面阻滞在妇科恶性肿瘤根治术中的应用研究

摘　　要：目的研究耳穴磁疗法联合腹横肌平面(TAP)阻滞在妇科恶性肿瘤根治术中的应用价值。方法选取2022年7月至2023年12月江西省妇幼保健院90例行妇科恶性肿瘤根治术的患者为研究对象,采用随机数字法分为空白组(行全麻手术)、耳穴贴磁组(予以耳穴贴磁)、联合组(予以耳穴贴磁联合TAP阻滞)各30例,比较三组术后恢复情况,镇痛泵按压次数、视觉模拟评分法(VAS)评分。结果三组的手术时间、术中出血量、尿量、血糖比较,差异无统计学意义(P>0.05),耳穴贴磁组、联合组的术中阿片类药物用量低于空白组,而耳穴贴磁组的术中阿片类药物用量高于联合组,差异有统计学意义(P<0.05);耳穴贴磁组、联合组的排气时间、术后排便时间、住院时间短于空白组,耳穴贴磁组、联合组术后72 h睡眠时间长于空白组,差异有统计学意义(P<0.05)。三组镇痛泵按压次数术后24、48 h低于本组术后12 h,空白组、联合组术后48 h低于本组术后24 h,差异有统计学意义(P<0.05)。联合组术后12、24、48 h镇痛泵按压次数少于耳穴贴磁组及空白组,差异有统计学意义(P<0.05)。联合组术后12、24 h VAS评分低于本组术后6 h,差异有统计学意义(P<0.05);空白组、耳穴贴磁组术后24 h VAS评分低于本组术后6、12 h,差异有统计学意义(P<0.05);联合组术后6、12、24 h VAS评分低于耳穴贴磁组及空白组,差异有统计学意义(P<0.05)。结论耳穴磁疗法联合TAP阻滞应用在妇科恶性肿瘤根治术中可明显降低镇痛药物用量,促进术后恢复,同时减少镇痛泵按压次数及疼痛,值得在临床推广实践。Objective To study the application value of auricular point magnetic therapy combined with transversus abdominis plane block(TAP)block in radical operation of gynecological malignant tumor.Methods A total of 90 patients who underwent radical surgery for gynecological malignant tumors in Jiangxi Maternal and Child Health Hospital from July 2022 to December 2023 were selected as the research objects,and they were divided into blank group(receiving surgery under general anesthesia),auricular acupoint magnetic paste group(receiving auricular acupoint magnetic paste),and combined group(receiving auricular acupoint magnetic paste combined with TAP block)according to the random number table method,with 30 cases in each group.The postoperative recovery,the number of analgesic pump and score of visual analogue scale(VAS)of the three groups were compared.Results There were no statistically significant differences in operation time,intraoperative blood loss,urine volume and intraoperative blood glucose among the three groups(P>0.05).The intraoperative opioid consumption of the auricular acupoint magnetic paste group and combined group were lower than that of blank group,while the intraoperative opioid consumption of the auricular acupoint magnetic paste group was higher than that of the combined group,with statistical significances(P<0.05).The exhaust time,postoperative defecation time and hospital stay in auricular acupoint magnetic paste group and combined group were shorter than those in blank group,and the sleep time at 72 h after surgery in auricular acupoint magnetic paste group and combined group were longer than that in blank group,the difference were statistically significants(P<0.05).The number of analgesic pump in the three groups at 24 and 48 h after surgery were lower than those at 12 h after surgery,and the number in blank group and combined group at 48 h after surgery were lower than those at 24 h after surgery,the differences were statistically significant(P<0.05).The number of analgesic pump in combined gro
