摘 要:目的分析宫颈癌淋巴结转移(LNM)患者动态对比增强磁共振成像(DCE-MRI)技术参数与宫颈癌组织中Hippo-YAP通路yes相关蛋白1(YAP1)表达的相关性。方法选取86例行DCE-MRI检查的宫颈癌患者为研究对象,根据病理检查结果分为LNM阳性27例,LNM阴性59例。利用血流动力学双室Tofts模型,获取宫颈癌原发灶的DCE-MRI定量参数[转移常数(K^(trans))、回流速率常数(K_(ep))、血管外细胞外间隙容积分数(V_(e))]。采用逆转录聚合酶链式反应(RT-PCR)法检测YAP1 mRNA表达。比较LNM阳性和阴性患者DCE-MRI定量参数以及YAP1 mRNA表达,Pearson相关性分析分析宫颈癌LNM阳性患者DCE-MRI定量参数与YAP1 mRNA表达的关系,受试者工作特征曲线(ROC)曲线评估K^(trans)、K_(ep)、V_(e)值与YAP1 mRNA表达对宫颈癌LNM的预测价值。结果LNM阳性组K^(trans)、K_(ep)值以及YAP1 mRNA表达显著高于LNM阴性组,差异有统计学意义(P<0.05)。Pearson相关性结果显示,宫颈癌LNM阳性患者K^(trans)、K_(ep)值与YAP1 mRNA表达呈正相关(P<0.05)。ROC曲线结果显示,K^(trans)、K_(ep)值和YAP1 mRNA表达单独及联合检测预测宫颈癌LNM的ROC曲线曲线下面积(AUC)分别为0.746、0.730、0.734、0.880,且联合检测的AUC较高(P<0.05)。结论宫颈癌原发病灶DCE-MRI定量参数(K^(trans)、K_(ep)值)以及Hippo-YAP通路YAP1表达与患者LNM相关,且K^(trans)、K_(ep)值与YAP1 mRNA表达呈正相关,三项指标联合检测可有效预测宫颈癌LNM的发生。Objective To analyze the correlation between dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)parameters and expression of HiPO-YAP pathway yes-associated protein 1(YAP1)in patients with lymph node metastasis(LNM)of cervical cancer.Methods A total of 86 patients with cervical cancer who underwent DCE-MRI examination were enrolled as the research objects.According to the results of pathological examination,there were 27 cases with LNM positive and 59 cases with LNM negative.DCE-MRI quantitative parameters[transfer constant(K^(trans)),reflux rate constant(K _(ep)),extravascular extracellular volume fraction(V_(e))]in primary cervical cancer were obtained by hemodynamics two-chamber Tofts model.The expression of YAP1 mRNA was detected by RT-PCR.DCE-MRI quantitative parameters and expression of YAP1 mRNA in LNM-positive patients and LNM-negative patients were compared.The relationship between DCE-MRI quantitative parameters and expression of YAP1 mRNA in LNM-positive patients was analyzed by Pearson correlation analysis.The predictive value of K^(trans),K _(ep),V_(e)and YAP1 mRNA for LNM was evaluated by ROC curves.Results K^(trans),K _(ep) and expression of YAP1 mRNA in LNM-positive group were significantly higher than those in LNM-negative group(P<0.05).The results of Pearson correlation analysis showed that K^(trans) and K _(ep) were positively correlated with expression of YAP1 mRNA in LNM-positive patients(P<0.05).The results of ROC curves analysis showed that area under the curve(AUC)values of K^(trans),K _(ep),YAP1 mRNA and combined detection for predicting LNM of cervical cancer were 0.746,0.730,0.734 and 0.880,respectively.AUC of combined detection was greater(P<0.05).Conclusion DCE-MRI quantitative parameters(K^(trans),K _(ep))in primary cervical cancer and YAP1 expression in HiPO-YAP pathway are correlated with LNM.K^(trans) and K _(ep) are positively correlated with expression of YAP1 mRNA.Combined detection of these three indexes can effectively predict the occurrence of LNM in cervical cance
