摘 要:目的探讨炎症相关标志物对初诊结肠癌严重程度的预测作用。方法回顾2020年1月29日至2020年12月13日于山西省人民医院肛门结直肠中心住院并行手术治疗明确为结肠癌的患者,共收集212例患者的资料,寻找炎症相关标志物:外周血单核细胞/淋巴细胞比值(MLR)、外周血中性粒细胞/淋巴细胞比值(NLR)、和外周血血小板/淋巴细胞比值(PLR)与肿物大小、淋巴结转移、肝转移的关系、美国癌症联合会(AJCC)分期,以及与肿瘤标志物:癌胚抗原(CEA)、糖类抗原199(CA199)、糖类抗原CA125水平之间的关系。结果①初诊的212例结肠癌患者中男性、年龄>60岁占一半以上,肿物>5 cm患者炎症相关标志物MLR、NLR和PLR明显升高(P值均<0.05)。存在淋巴结转移患者炎症相关标志物MLR、NLR和PLR升高(P值均<0.05)。MLR、NLR和PLR数值在AJCC分期高的组高于AJCC分期组的低(P值均<0.05)。②炎症相关标志物(MLR、NLR和PLR)与肿瘤标志物具有相关性,MLR与CEA、CA199、CA125水平相关(P值均<0.05)。NLR与CEA、CA199、CA125水平相关(P值均<0.05)。PLR与CEA、CA199、CA125水平亦相关(P值均<0.05)。结论炎症标志物MLR、NLR和PLR组合可作为新诊断的结肠癌者肿瘤特征的工具。Objective To investigate the predictive effect of inflammation-related markers on the severity of colon cancer in the first diagnosis.Methods Patients with colorectal cancer admitted to the Colorectal Center of Shanxi Provincial People′s Hospital from January 29,2020 to December 13,2020.A total of 212 patients were collected.Study the relationship between inflammation-related markers of peripheral blood monocyte-to-lymphocyte(MLR),neutrophil-to-lymphocyte ratio(NLR),and platelet-to-lymphocyte ratio(PLR)and swelling size,lymph node metastasis,liver metastasis,AJCC phase and relationships with tumor markers carcinoembryonic antigen(CEA),carbohydrate antigen(CA)199,carbohydrate antigen(CA)125 levels.Results①More than half of the 212 cases of colon cancer were male and older than 60 years old,and the inflammation-related marker of MLR,NLR,and PLR increased significantly in patients with swelling greater than 5cm(P<0.05).Inflammation-related marker of MLR,NLR,and PLR were elevated in patients with lymph node metastasis(P<0.05).MLR,NLR,and PLR are significantly higher in the AJCC high group than in the AJCC low group(P<0.05).②Inflammation-related markers(MLR,NLR,and PLL)are positively related to tumor markers.MLR is positively related to CEA,CA199,and CA125 levels(P<0.05).The NLR is associated with CEA,CA199,and CA125(P<0.05).The PLR is also related to CEA,CA199,and CA125(P<0.05).Conclusion The combination of inflammatory markers MLR,NLR,and PLR in colon cancer patients can be used as tools for newly diagnosed tumor characteristics.
