摘 要:目的探讨慢性牙周炎合并牙列缺损患者口腔种植术后龈沟液成骨相关因子水平变化及其与种植体预后的关系。方法选取本院接受口腔种植术的慢性牙周炎合并牙列缺损患者92例(慢性牙周炎组)以及同期接受口腔种植术的牙周健康的牙列缺损患者60例(对照组);检测两组患者术后1周、2周和1月的龈沟液成骨相关因子碱性磷酸酶(ALP)、Runt相关转录因子2(RUNX2)、骨钙素(OCN)及骨保护素(OPG)水平并分析其与种植体预后的关系。结果慢性牙周炎组术后2周和1月时龈沟液ALP、RUNX2、OCN及OPG水平明显低于对照组(P<0.05)。预后良好组术后2周和1月时龈沟液ALP、RUNX2、OCN和OPG水平明显高于预后不良组(P<0.05)。多因素Logistic回归分析显示,术后2周的ALP(OR=0.771,95%CI:0.598~0.993)、OPG(OR=0.917,95%CI:0.842~0.999)及术后1月的RUNX2(OR=0.828,95%CI:0.719~0.953)是预后不良的独立影响因素(P<0.05),其预测预后不良的ROC曲线下面积分别为0.765、0.728和0.820。结论慢性牙周炎合并牙列缺损患者口腔种植术后龈沟液成骨相关因子变化趋势是先增加后降低,且术后2周的ALP、OPG和术后1月RUNX2水平是种植体预后不良的独立影响因素。Objective To investigate changes of osteogenesis-related factors in gingival crevicular fluid and their correlation with implant prognosis in patients with chronic periodontitis and dentition defect after oral implantation.Methods 92 patients with chronic periodontitis and dentition defects who received dental implant surgery(chronic periodontitis group)and 60 patients with periodontal healthy dentition defects who received dental implant surgery(control group)in our hospital were selected.The levels of osteogenesis-related factors alkaline phosphatase(ALP),runt-related transcription factor 2(RUNX2),osteocalcin(OCN)and osteoprotegerin(OPG)in gingival crevicular fluid were detected at 1 week,2 weeks and 1 month after operation,and the relationship between them and the prognosis of implants was analyzed.Results The levels of ALP,RUNX2,OCN and OPG in gingival crevicular fluid in the chronic periodontitis group were significantly lower than those in the control group at 2 weeks and 1 month after surgery(P<0.05).The levels of ALP,RUNX2,OCN and OPG in gingival crevicular fluid in the good prognosis group were significantly higher than those in the poor prognosis group at 2weeks and 1 month after surgery(P<0.05).Multivariate Logistic regression analysis showed that the ALP(OR=0.771,95%CI:0.598-0.993)and OPG(OR=0.917,95%CI:0.842-0.999)at 2 weeks after surgery,and RUNX2(OR=0.828,95%CI:0.719-0.953)at 1 month after surgery was an independent factor of poor prognosis(P<0.05),and the areas under the ROC curve for predicting poor prognosis were 0.765,0.728 and 0.820,respectively.Conclusion The changes trend of osteogenesis-related factors in gingival crevicular fluid in patients with chronic periodontitis and dentition defect after oral implantation were first increased and then decreased.The levels of ALP and OPG at 2 weeks after surgery and RUNX2 at 1 month after surgery were independent factors for the poor prognosis of implants.
