摘　　要：目的分析Bowen's病的临床特点、组织学特征及治疗方案,为Bowen's病临床诊治提供参考。方法收集2010年10月—2022年7月扬州大学附属苏北人民医院收治的74例Bowen's病的临床资料进行回顾性分析,总结该病患病年龄、皮损症状及病理表现等特点。结果74例Bowen's病例中男性31例,女性43例,发病年龄43~89岁,其中大于60岁患者61例。所有患者均为单发,其中四肢27例,面颈部27例,躯干12例,会阴及生殖器8例。48例患者无自觉症状,26例患者自觉瘙痒或疼痛。初次诊断与病理诊断一致为45例,误诊29例。所有患者病理均表现为鳞状上皮不同程度异型增生,手术切除后1年随访无复发。结论Bowen's病临床易误诊,需结合临床特点及病理检查确诊,手术切除仍为其主要治疗方式。Objective To retrospectively analyze the clinical features,histological features and treatment plan of 74 cases of Bowen's disease,so as to provide reference for the clinical diagnosis and treatment of Bowen's disease.Methods The clinical data of 74 cases of Bowen's admitted to Northern Jiangsu People's Hospital Affiliated to Yangzhou University from October 2010 to July 2022 were collected,and the clinical characteristics of Bowen's disease,such as age,skin lesions and pathological manifestations,were summarized.Results Among 74 Bowen's cases,31 were males and 43 were females.The age of onset ranged from 43 to 89 years old,among which 61 cases were over 60 years old.All patients were single,with 27 cases of limbs,27 cases of face and neck,13 cases of trunk,and 8 cases of perineum and genitalia.48 patients had no symptoms and 26 patients had pruritus or pain.The initial diagnosis was consistent with pathological diagnosis in 45 cases,and misdiagnosed in 29 cases.All patients showed different degrees of dysplasia of squamous epithelium in their pathology.No recurrence was observed at 1-year follow-up.Conclusion Bowen's disease is easy to be misdiagnosed in clinic,which needs to be combined with clinical characteristics and pathological examination.Surgical resection is still the main treatment method.
