2019~2023年广州某眼科医院眼部真菌感染病原学分析

摘　　要：目的分析广州某眼科医院近5年眼部真菌感染致病菌的分布和特征。方法对2019年1月~2023年12月在广州某眼科医院就诊且真菌培养结果为阳性的984例患者的性别、年龄和菌属分布等资料进行回顾性分析。结果984例眼部真菌感染患者中,男612例,女372例,男女比例为1.65﹕1;41~60岁的中年人是真菌感染的主要人群,共518例,占比52.64%。真菌培养阳性菌株数前5位的菌属依次是镰刀菌属540株(54.88%)、曲霉菌属165株(16.77%)、刺盘孢属65株(6.60%)、念珠菌属62株(6.30%)和链格孢属38株(3.86%);镰刀菌属占比过半,是最主要的单一致病菌属。眼部真菌感染的主要感染部位为角膜,共有890例(90.45%)。角膜真菌感染的致病菌以镰刀菌属为主,共有525株(58.99%)。眼内结构中,玻璃体和房水真菌感染的致病菌均以念珠菌属为主,分别为23株(47.92%)和6株(33.33%)。结论广州某眼科医院眼部真菌感染患者在年龄、性别、菌属分布等方面的特征与国内外报告相似,但也有罕见真菌(刺盘孢属)感染病例多的自身特点。Objective To analyze the distribution and characteristics of pathogenic bacteria of ocular fungal infection in an eye hospital in Guangzhou in recent 5 years.Methods A total of 984 cases of positive fungal culture in an eye hospital in Guangzhou from January 2019 to December 2023 were retrospectively analyzed for gender,age and bacterial species distribution.Results Among the 984 patients with ocular fungal infection,612 were males and 372 were females,the male to female ratio was 1.65∶1.The middle-aged people aged 41~60 years were the main population of fungal infection,with 518 cases,accounting for 52.64%.The top 5 genera of fungal culture positive strains were 540 strains of Fusarium(54.88%),165 strains of Aspergillus(16.77%),65 strains of Colletotrichum(6.60%),62 strains of Candida(6.30%)and 38 strains of Streptospora(3.86%).Fusarium accounted for more than half of the total,which was the most important single pathogen.Cornea was the main site of ocular fungal infection,with a total of 890 cases(90.45%).Fusarium was the main pathogen of corneal fungal infection,with a total of 525 strains(58.99%).In the intraocular structure,Candida was the main pathogen of vitreous and aqueous humor fungal infection,with 23 strains(47.92%)and 6 strains(33.33%),respectively.Conclusion The characteristics of ocular fungal infection patients in an eye hospital in Guangzhou were similar to those reported at home and abroad in terms of age,gender,strain distribution,etc.,but there were also a rare fungal(Colletotrichum)infection cases of their own characteristics.
