摘　　要：目的:评价CT直方图分析(CT histogram analysis,CTHA)方法在原发性肝细胞肝癌(Hepatocellular carcinoma,HCC)的诊断价值。方法:本研究采用回顾性分析方法,时间2021年6月至2022年9月,共纳入经手术病理确诊的HCC患者60例(男/女=50/10,平均年龄47.10±9.31岁),所有病例术前接受腹部三期CT增强扫描,包括动脉期、静脉期和延迟期。两位放射医师分别使用GE公司Ommi-Kinetics,Version V2.1.0 R软件对图像进行处理并获取直方图参数,采用Logistic回归和受试者工作特征曲线,评价直方图参数及多参数联合对HCC的诊断效能。结果:动脉期参数Kurtosis、Relative deviation,静脉期参数Uniformity、Relative deviation,延迟期参数Relative deviation可作为独立预测因子用于HCC的诊断,多参数联合的诊断效能最高(曲线下面积=0.989)。结论:CTHA方法可于HCC的诊断。Purpose:To assess the performance of computed tomography histogram analysis(CTHA)for the diagnosis of hepatocellular carcinoma(HCC).Methods:From June 2021 to September 2022,this retrospective study enrolled 60 patients with confirmed histopathological HCC(50 men,10 women;mean age 47.10±9.31 years)who had undergone preoperative contrast-enhanced CT imaging before surgery.Two radiologists independently segmented lesions for quantitative histogram analysis.Histogram features were extracted from arterial,venous and delayed phase CT images using GE company software(Ommi-Kinetics,Version V2.1.OR).Logistic regression and receiver operating characteristic(ROC)analysis were used to identify histogram parameters to discriminate HCC from non-tumor hepatic tssue.Results:There were significant differences between HCC and non-tumor hepatic tissue on histogram features:Several histogram features can serve as independent predictors including Kurtosis,Relative deviation,and Uniformity.Areas under the curve of the combined parameters were 0.989.Conclusion:CT histogram analysis can be used as an auxiliary diagnostic method for HCC.
