外周血T细胞亚群及血清白细胞介素水平与早期胃癌患者内镜黏膜下剥离术后并发感染的相关性分析

摘　　要：目的探究外周血T细胞亚群、血清白细胞介素(IL)水平与早期胃癌患者内镜黏膜下剥离术(ESD)后并发感染的相关性。方法回顾性抽取2021年1月至2023年1月于新乡医学院第一附属医院行ESD治疗的106例早期胃癌患者,根据术后感染情况分为感染组(26例)和未感染组(80例)。检测并比较两组患者血清IL及外周血T细胞亚群的表达水平。通过受试者工作特征曲线评价血清IL及外周血T细胞亚群表达水平对患者术后感染的预测价值。结果感染组CD4^(+)T细胞比例、CD4^(+)/CD8^(+)、IL-10水平低于未感染组,CD8^(+)T细胞比例、IL-8及IL-12水平高于未感染组(P<0.05)。CD4^(+)T细胞比例、CD4^(+)/CD8^(+)及血清IL-10、IL-8水平对早期胃癌患者ESD术后感染发生均具有较高预测效能,且CD4^(+)/CD8^(+)对术后感染的预测效能最高(P<0.05)。结论早期胃癌患者ESD术后感染的发生与外周血CD4^(+)T细胞比例、CD4^(+)/CD8^(+)及血清IL-8、IL-10水平相关,及时监测患者外周血T细胞水平及血清IL水平对于早期评估、预防术后感染具有一定的临床意义。Objective To investigate the correlations of levels of T cell subsets in peripheral blood and interleukin(IL)in serum with occurrence of infection after endoscopic submucosal dissection(ESD)for early gastric cancer.Methods A total of 106 patients with early gastric cancer treated by ESD in the First Affiliated Hospital of Xinxiang Medical University from January 2021 to January 2023 were retrospectively selected,and they were divided into the infection group(26 cases)and the non-infection group(80 cases)according to occurrence of postoperative infection.The levels of serum IL and T cell subsets in peripheral blood of the two groups were detected and compared.Receiver operating characteristic curve was used to evaluate the predictive value of the levels of serum IL and T cell subsets in peripheral blood for postoperative infection.Results The proportion of CD4^(+)T cells,CD4^(+)/CD8^(+),and IL-10 level in the infection group were lower than those in the non-infection group,while the proportion of CD8^(+)T cells,IL-8 level,and IL-12 level were higher than those in the non-infection group(P<0.05).The proportion of CD4^(+)T cells,CD4^(+)/CD8^(+),and serum IL-10 and IL-8 levels have high predictive efficiency for postoperative infection in early gastric cancer patients after ESD,in addition,CD4^(+)/CD8^(+)had the highest predictive efficacy for postoperative infection(P<0.05).Conclusions The incidence of infection after ESD in patients with early gastric cancer is correlated with the proportion of CD4^(+)T cells,CD4^(+)/CD8^(+),and the levels of IL-8 and IL-10 in serum.Monitoring the levels of T cells and IL in peripheral blood in time has certain clinical significance for early evaluation and prevention of infection after ESD in patients with early gastric cancer.
