高频小探头超声内镜对食管癌及胃癌T分期的诊断价值

摘　　要：目的探讨高频小探头超声内镜(EUS)对食管癌及胃癌T分期的诊断价值。方法回顾性抽取2018年1月至2023年1月河南省中医院收治的经内镜及病理检查明确诊断为食管癌或胃癌的患者90例,均于术前接受EUS检查。以手术病理结果为金标准,分析EUS与手术病理检查对食管及胃恶性肿瘤T分期的一致性。结果31例食管癌患者中,病理诊断T1期4例,T2期8例,T3期15例,T4期4例;EUS诊断T分期(T1、T2、T3、T4)分别为5、10、13、3例;EUS诊断与病理诊断的一致性Kappa值(T1、T2、T3、T4)分别为0.626、0.377、0.481、0.581。59例胃癌患者中,病理诊断T1期20例,T2期19例,T3期18例,T4期2例;EUS诊断T分期分别为(T1、T2、T3、T4)19、22、14、4例;EUS诊断与病理诊断的一致性Kappa值(T1、T2、T3、T4)分别为0.655、0.441、0.574、0.302。结论EUS操作简便,在食管癌、胃癌T分期诊断中有着重要的应用价值。Objective To investigate the diagnostic value of high frequency small probe endoscopic ultrasonography(EUS)for T staging of esophageal carcinoma and gastric carcinoma.Methods A total of 90 patients diagnosed with esophageal cancer or gastric cancer by endoscopy and pathology were retrospectively selected from Henan Provincial Hospital of Traditional Chinese Medicine from January 2018 to January 2023.They all underwent EUS examination before surgery.Using surgical pathology as the gold standard,the consistency of EUS with surgical pathology examination in the T staging of esophageal carcinoma and gastric malignant tumors was analyzed.ResultssAmong the 31 patients with esophageal carcinoma,4 cases were pathologically diagnosed with T1 stage,8 cases were diagnosed with T2 stage,15 cases were diagnosed with T3 stage,and 4 cases were diagnosed with T4 stage.Among the 31 patients with esophageal carcinoma,EUS identified 5 cases with T1 stage,10 cases with T2 stage,13 cases with T3 stage and 3 cases with T4 stage.The consistent Kappa values of EUS with pathology in the diagnosis of esophageal carcinoma(stages T1,T2,T3,and T4)were 0.626,0.377,0.481,and 0.581,respectively.Among the 59 patients with gastric carcinoma,pathology examination identified 20 cases with T1 stage,19 cases with T2 stage,18 cases with T3 stage,and 2 cases with T4 stage.Among the 59 patients with gastric carcinoma,EUS identified 19 cases with T1 stage,22 cases with T2 stage,14 cases with T3 stage,and 4 cases with T4 stage.The consistent Kappa values of EUS with pathology in the diagnosis of gastric carcinoma(stages T1,T2,T3,and T4)were 0.655,0.441,0.574,and 0.302,respectively.Conclusions EUS has the advantage of simple and feasible surgical procedure,and it has important application value in the diagnosis of T stage of esophageal carcinoma and gastric carcinoma.
