摘  要:目的:分析骨化三醇联合阻抗训练治疗老年原发性肌少症的效果。方法:选取2018年2月—2022年12月上海市健康医学院附属周浦医院收治的老年原发性肌少症患者70例作为研究对象,按照随机数字表法分为观察组与对照组,每组35例。对照组实施常规健康干预,观察组在对照组基础上给予骨化三醇治疗。比较两组肌少症相关指标、血清维生素D、肌肉生长抑制素(MSTN)水平。结果:治疗前,两组四肢骨骼肌量指数(ASMI)、握力(HGS)、骨骼肌功能评分比较,差异无统计学意义(P>0.05);治疗后,两组ASMI、HGS、SPPB评分高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。治疗前,两组清25-羟维生素D3[25-(OH)D3]、MSTN水平比较,差异无统计学意义(P>0.05);治疗后,两组MSTN水平低于治疗前,且观察组低于对照组,两组25-(OH)D3水平高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:骨化三醇联合阻抗训练治疗老年原发性肌少症的效果显著,可改善患者肌肉质量、力量、骨骼肌功能,提高25-(OH)D3水平,降低MSTN表达。Objective:To analyze the effect of calcitriol combined with impedance training on primary sarcopenia in the elderly.Methods:Seventy elderly patients with primary sarcopenia admitted to Zhoupu Hospital Affiliated to Shanghai Medical College of Health from February 2018 to December 2022 were selected as the study objects,and were divided into observation group and control group according to random number table method,with thirty-five cases in each group.The control group received routine health intervention,and the observation group received calcitriol treatment on the basis of the control group.The levels of serum vitamin D and myostatin(MSTN)were compared between the two groups.Results:Before treatment,there were no significant differences in skeletal muscle mass index(ASMI),grip strength(HGS)and skeletal muscle function scores between the two groups(P>0.05);after treatment,ASMI,HGS and SPPB scores in 2 groups were higher than before treatment,and observation group was higher than control group,the difference was statistically significant(P<0.05);before treatment,there was no significant difference in the levels of 25-hydroxyvitamin D3[25-(OH)D3]and MSTN between the two groups(P>0.05);after treatment,the MSTN level in both groups was lower than before treatment,and the observation group was lower than the control group,and the 25-(OH)D3 level in both groups was higher than before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:Calcitriol combined with impedance training can improve muscle mass,strength and skeletal muscle function,increase 25-(OH)D3 level and decrease MSTN expression in elderly patients with primary sarcopenia.
